Unit Summary:
The HHS remote patient monitoring (RPM) and virtual nursing program supports patients across all HHS sites through virtual care and remote patient monitoring. The program, based at the PROTECT Lab at Juravinski Hospital, aims to prevent complications during recovery and empower patients to define their recovery goals. Expansion to other patient populations is anticipated.
Position Summary:
Reporting to the Unit Manager and Clinical Leader, the Registered Nurse in the Virtual Nursing Station-Surgical Transitions provides patient care aligned with the College of Nurses of Ontario standards and HHS policies. Responsibilities include conducting virtual visits, follow-up via video or phone, scheduling, and patient education. The role involves collaboration with an interdisciplinary team and visiting patients and families in hospital to provide program information.
Schedule Work Hours:
Days, evenings, weekends, holidays, and on-call night coverage.
Qualifications:
- Current Certificate of Registration with the College of Nurses of Ontario.
- Recent (5-7 years) clinical experience in medical-surgical nursing.
- Leadership and management skills.
- Excellent communication, team-building, and change management abilities.
- Effective written and verbal communication skills.
- Interpersonal, public relations, and problem-solving skills.
- Ability to prioritize and organize work.
- Ability to collaborate effectively across hospital departments.
- Membership in good standing with the College of Nurses of Ontario.
- Experience with virtual care preferred.
- Experience with EPIC workflows and EMR best practices preferred.
