Detailed OverviewSupporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families:
Provides clinical program leadership and management of the mental health & substance use residential care and supported housing program supporting the residents, families and health service providers of residential licensed facility sites contracted by Fraser Health and supported housing contracted programs, and incorporates and promotes a trauma-informed and recovery-oriented approach and harm reduction framework while managing contract performance management and quality initiatives.
Accountabilities include supporting contracted service providers to meet contractual compliance, including compliance with Ministry of Health policy, in collaboration with Community Care Facilities licensing for facilities under the Community Care and Assisted Living Act and through work with Service Providers to support the delivery of safe, appropriate, and quality care to residents; supporting regional clinicians in providing residential liaison/coordination activities including complaints management and investigation; responding to and processing requests received by contracted services providers under the Freedom of Information and Protection of Privacy Act; and, ensuring the communication and resolution of system issues. Responsible for investigating quality of care concerns across contracted service provider beds as designated by senior management; develops recommendations to address findings/issues; maintains tracking and trending of quality-related information. Conducts Quality of Service Reviews on behalf of Fraser Health; leads and directs the development of Fraser Health wide facility activity requirements and special projects to expand residential capacity in contracted services.
Responsibilities
- Provides clinical operational management of existing contracts and services for facilities/service providers of MHSU residential services within Fraser Health.
- Develops and implements priority plans for the program, as well as goals, objectives, policies and procedures, ensuring effective outcome based services to build residential capacity in contracted MH&SU residential care facilities.
- Develops and manages an interdisciplinary team that provides clinical oversight; liaison; coordination and case management to supports the needs of clients and health service providers in contracted facilities. Maintains close linkages with regional referral team regarding access and flow.
- Selects, directs, supervises and evaluates staff to ensure performance of expectations/duties; promotes, disciplines & terminates staff as required.
- Provides consultation to the team regarding resolution of complex care issues, and interpretation of Fraser Health, Ministry, Licensing, Adult Guardianship and relevant Acts.
- Collaborates with internal and external partners to provide a comprehensive and integrated approach to clinical quality and contractual oversight in contracted residential services.
- Identifies and recommends the development of change of assigned program(s) to address current and future issues. Liaises with internal programs such as Access, Licensing, Home Health, Finance and Quality Improvement to discuss and resolve issues and to insure coordinated and responsive approaches.
- For operational/clinical/system issues, identifies areas of significant risk; coordinates investigation/follow up and develops mitigation strategies.
- Conducts regular contract reviews, addresses issues of non-compliance and implements mitigating strategies. Negotiates contracted compliance regarding MOH policy on a variety of issues (ie outbreak protocols) for existing contracts.
- Manages annual operating budgets for the contracted programs ensuring optimum utilization of funds.
- Plans and manages administration and information systems and resource plans so that daily operations and support activities are conducted in a timely, effective and outcome based approach.
- Leads and directs the development of facility activity requirements (ie. data collection, rate verification, temporary beds; leads and/or conducts special projects).
- Participates in MHSU Services and Fraser Health-wide planning for MHSU residential and related services. Liaises with user groups to discuss and identify strategies, initiates contact with external agencies and other health authorities to identify areas for opportunities and change.
- Liaises with Managers, Administrators, Physicians and other agencies to promote communications, information sharing and soliciting feedback on initiatives.
- Coordinates and/or prepares program statistics and information as required for program evaluation and improvement.
- Provides leadership on an annual/regular basis to initiatives or projects within MHSU Residential Contracts & Services, including a focus on capacity-building of contracted services to meet the needs of patients/clients/residents and families.

QualificationsEducation and ExperienceBachelor's degree in a health related discipline, plus seven (7) years' experience in a community health care setting including five (5) years related clinical management experience with residential care facilities, or an equivalent combination of training and experience.
Valid Class V B.C. Driver's License with access to a vehicle for business related purposes.
