Pay Range OPSEU/VHA Pay Level 4B—$24.44 per hour The Scheduling Coordinator provides scheduling functions by ensuring healthcare services are scheduled in a timely manner. What will be your role? The Coordinator is a unique integrated coordinator role at VHA Home Healthcare and responsible for the scheduling and information exchange for personal support, nursing and rehab professionals as part of this innovative East Toronto Health Partners’ (ETHP) strategy to improve client transitions from hospital-to-home through the established MGH2Home program. Reporting to the MGH2Home Program Supervisor, the Coordinator supports rapid transitions of clients and families by communicating effectively with our clients, hospital partner and health care providers to enable effective service utilization. This is a fast-paced program, and the Scheduling Coordinator is critical to supporting clients and families who rely on care 7 days per week. The role involves preparing for new client referrals from the hospital, communication with clients and families, initiating and updating service plans, responding to scheduling requests and changes and regular communications with the hospital and service providers (Personal Support Workers, Nurses, and Rehabilitation). The Coordinator is also responsible for inputting accurate information for data collection purposes for the program – through VHA’s secure online platforms. This role may involve being cross trained with other integrated care programs within VHA to support a consistent and stable coverage model. Required Qualifications: Post-Secondary School Education: a certificate/diploma in healthcare education is preferred Minimum of three years related work experience in a healthcare organization/agency (preferably in the community) or in an administrative position that required extensive scheduling; experience of multi-disciplinary scheduling health care providers in the community sector would be an asset Empathy-based and person-centered telephone communication skills with the ability to actively listen and to respond effectively to crucial conversations that require escalation to a supervisor/manager Strong organizational skills with attention to detail and the ability to prioritize multitasking and quickly adjust to change Excellent communication and interpersonal skills Experience working within an interprofessional or integrated care approach is an asset Knowledge of the GTA and TTC routes or relevant contracted areas What makes VHA’s compensation unique?
