Job DescriptionAssesses the dietary end-of-life needs unique to each person and family, considering the social determinants of health, ethnicity, culture, gender, sexual orientation, language, religion, age, ability and their preferences. Assesses the person’s and caregivers’ skills and need for dietary education and supports Creates a holistic, person-centred plan, acknowledging the psychosocial impact of changing nutritional requirements and dietary intakeProvides education, advice and practical strategies to the person and caregivers regarding the management of dietary related symptoms, including anorexia, cachexia, dysphagia, nausea, vomiting, constipation, diarrhea and taste changes, management of enteral and parenteral nutrition support Engages in dysphagia assessment and management Collaborate with interdisciplinary teams, including but not limited to physicians, nurse practitioners, nurses, personal support workers, social workers, speech therapists, occupational therapists, and physiotherapists. Contributes to interdisciplinary team meetings and participates in care planning conferences QualificationsCompletion of a Bachelor’s or Master’s Degree in an accredited Food & Nutrition program Registration with the College of Dietitians of Ontario required Member in good standing with the Dietitians of Canada preferred Minimum 3-5 years' experience working as a Registered Dietitian Successful completion of any of the following is considered an asset:? Learning Essentials Approaches to Palliative and End of Life Care (LEAP); or? Fundamentals of Palliative Care? The Comprehensive Advanced Palliative Care Education (CAPCE)?
