Job Description: Position Purpose summary: The Shift Lead LPN is a working lead position with limited supervision. This position focuses on the operational aspects of resident care ensuring that care plans are implemented effectively. The Shift Lead LPN is responsible for the assessment, planning, implementation and evaluation of resident care. The Shift Lead LPN provides support and direction to the Licensed Practical Nurses and the Health Care Aides. The Shift Lead LPN is an active member of the interdisciplinary team and is accountable for providing safe and competent nursing care within the scope of LPN practice. All activities carried out by the Shift Lead LPN are consistent with The Salvation Army’s mission and values as well as policies, procedures and protocols of The Edmonton Centre of Hope. The Shift Lead LPN working at Grace Manor must demonstrate knowledge and be familiar with care of mental health and geriatric clients. Key Accountabilities: Lead Supervisory Duties Ensure the ongoing monitoring of care is delivered according to the resident care plan. Collaborate with Manager, Educator, Director of Care and Alberta Health Services Case Managers with the implementation and evaluation. Develop structures and processes which integrate evidence-informed nursing practice by analyzing, interpreting and using information to support decisions int eh practice environment. Work with the Leadership team, care staff, and AHS professional team and physicians to discuss current initiatives and resolve issues related to improved care delivery Lead and supervise the interdisciplinary team to ensure effective, consistent and comprehensive resident care planning and delivery Assist with new staff orientation, staff performance appraisals as allocated Offer staff mentoring and education as required Facilitate communication between residents, families and staff, ensuring staff are informed and involved in resident person-centered care Act as shift supervisor responsible for leading staff shift change and report and answering questions regarding the shift report Responsible for staff shift allocation Act as Fire Warden in case of emergency May participate in recruitment when required Program Operations Assist in planning, developing, implementing, monitoring quality improvement and risk management processes as per Edmonton Centre of Hope policy. Identify and define barriers to quality resident care and provide innovative and collaborative solutions. Carry the LPN phone while on shift for ongoing communication with the care staff and to respond to emergent/urgent situations, providing solutions in collaboration with the leadership team Establish and maintain therapeutic relations with AHS, families, volunteers and others through effective interpersonal and interviewing techniques Maintain accurate records related to resident activities, incidents and staff performance, ensuring confidentiality and compliance with regulations Promote health and safety regulations including conducting regular safety checks and assisting in emergency procedures and preparedness Monitoring and Assessment Perform and contribute to the initial and ongoing nursing assessment of assigned Residents. Collect, document and report health information from objective/subjective sources to identify Resident’s health needs, monitor changes in health status using critical judgment skills, assess Resident’s ability to function independently in their home environment. Document timely, accurate reports of data collection, interpretation, planning, implementing and evaluating care in accordance with Salvation Army Policies & Procedures, and regulatory requirements. Care Planning & Implementation Analyze information and identifies priorities, initiates and/or collaborates in the development of the plan of care to meet Resident needs. Provide, initiate, and reinforce teaching using developed educational materials Participate in the development of a discharge plan to meet the Resident’s needs, including identifying and coordinating resources required to ensure continuity of care. Identify and initiate interventions within the LPN’s legislated scope of practice Implement nursing treatments and interventions using evidence-based knowledge to accomplish plan of care Evaluation & Documentation Observe, report and document changes in Resident status and deviations from anticipated response/outcome. Evaluate and suggest modification to plan of care in accordance with Resident care needs on an ongoing basis. Make recommendations to the Health Care Team, the AHS Case Manager, Social Worker and the Manager or designate regarding changes to the treatment plan based on an ongoing assessment of the Resident’s situation/status Assess and monitor unstable/ill residents throughout the shift and assumes care or consults with the Registered Nurse or Registered Psychiatric Nurse or arranges transfer to acute care if necessary. Communicate promptly with AHS Case Manager and/or physician when there is a change in a resident’s status Record complete and accurate relevant data for every Resident in a retrievable form according to departmental policies. Professional Responsibilities Participate in programs to develop and maintain clinical competency and promote professional growth. Participate as a team member in quality improvement activities and participates in nursing practice committees. Responsible for reporting unskilled or unprofessional conduct, and any behavior contrary to The Salvation Army standards of conduct and policies, by any member of the interdisciplinary team to the Registered Nurse or Registered Psychiatric Nurse and Manager at the first available opportunity. Education and Research Participate in research, staff development projects, in-service education classes and other activities as appropriate. Participate in the orientation of new employees/students, participate in precepting students as assigned. Occupational Health and Safety Co-operate and participate fully in the Occupational Health and Safety system at The Salvation Army. Identify and report hazards and incidents with the potential for injury to themselves or others to the supervisor Working Conditions: · Working environment is generally in the residential building in fast-paced conditions. The job requires lifting/twisting/bending and will require sitting and standing for long periods of time. · Shift work is required. · The incumbent’s work environment is typically at work sites within residential settings, and may experience the following disagreeable conditions: therefore, will need to follow or use universal precautions: o Dealing with angry and abusive people, verbal abuse o Exposure to bodily fluids, bodily waste, and infectious diseases o Exposure to drugs, alcohol, and solvents o Physical attacks by others o Exposure to toxic chemicals o Sharp objects (i.e., needle sticks, knives, and weapons) o Immunization for Hepatitis B and vaccination against Tuberculosis o Responding to first aid emergencies; including overdose and death Education, Qualifications and Certifications: · Graduate of an approved Licensed Practical Nurse’s Program. · Successful completion of an accredited post-basic study program as required within the scope of the assigned position. ·Annual proof of active registration with CLPNA. · Current certification in Basic Life Support · Satisfactory Criminal Police Investigation Check with Vulnerable Sector Check · Certification in Non-Violent Crisis Intervention Experience and Skilled Knowledge Requirements: · Minimum one year experience in a supervisory / lead position · Two years’ experience in a continuing care or mental health setting preferred. Attention to detail · Keen observation skills · Critical thinking skills · Proficiency with email, MSWord, and MS Excel preferred Permanent Full-Time, 1FTE, scheduled on a 4-week rotation consisting of day and evening shifts Target hiring range for this position is $31.39 per hour with a maximum of $41.09 per hour Compensation: The target hiring range for this position is $31.39 to $36.24 with a maximum of $41.09. Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
