Job Description: Program Engage in the development of inter-personal relationships that promotes dignity and respect Utilize approved case management tools, Outcome Star, SPDAT, and Pathway of Hope. Coordinate, refer and deliver all needed wraparound supports Conduct case work and follow-up services which includes but not limited to Intake Needs/Skills Assessment for Housing history Barriers to housing Immediate and ongoing needs (identification, financial, food, housing, clothing, medical and education) Conduct preliminary Individualized Goal Setting/Plan Provide Crisis Intervention Complete discharge plans and summaries Maintain contact with clients for the purpose of Monitoring progress of clients in working on goal plan Referrals to appropriate community resources, following established procedures, Advocate on behalf of client to Bayside staff and community resources as requested by Director of Program. Educate clients in health and safety concerns to achieve a safe living environment Maintain contact with client through regular visits Coach and support clients as needed to access appropriate services Identify individual strengths and assets to sustained tenancy Provide for basic life skills counseling and development Provide warm transfers to requested partner agency staff Collect statistical information related to the clients in program Administrative Responsibilities Prepare, keep, and maintain accurate case notes, records, and HIFIS database (intermittently throughout each workday) and share them to relevant individuals involved in the client’s circle of care while maintaining client’s confidentiality Prepare statistical and other reports as required Ensure that the privacy and confidentiality of clients is maintained within established standards, Policy and Procedures. Present case reports and progress of clients at regularly scheduled case management meetings Be involved in ongoing professional development and to attend Staff / Development meetings Work as a member of the team to ensure that the program operates efficiently and effectively (and to ensure that other staff’s responsibilities are covered when they are away) Participate in and represent the organization at external committees and forums Participate in internal committees and/or activities relating to the organization (e.g. Annual general meeting, other social recreation events, etc.) Service Responsibilities The incumbent will promote positive and supportive attitudes toward men and families who are experiencing homelessness Promote programs and activities that empower the men and families to work towards their individual goals. Act as an advocate and be a positive role model for the men and families in the community Develop effective working relationships and partnerships with community members, representatives of other departments, agencies, and co-workers Perform other related duties as requested from time to time by the Director of Programs Ability to undergo applicable screening and background checks successfully that are satisfactory to The Salvation Army, in its sole discretion and provide an original copy of a Background check with vulnerable sector screening. Ability and willingness to develop an understanding and support for the mission and purpose of The Salvation Army in Canada and its implications as related to position responsibilities. Requires continuously monitoring the environment and attentiveness when interacting with clients and recording information. May work with persons who become angry Caseworker carries a cell phone when off the building and wears a panic button when in the building. The above responsibilities must be performed in keeping with The Salvation Army’s Mission, Vision and Values, in a professional manner, upholding our code of conduct. Education, Qualifications and Certifications: • Degree or diploma in Social Services and/or equivalent education and experience. Note: An alternative level of education and experience may be acceptable. Completion of Non-Violence Crisis Intervention Training or willingness to obtain. Current First Aid and CPR or willingness to obtain. Experience and Skilled Knowledge Requirements Possess a minimum of two (2) years to a maximum of five (5) years of previous experience with people experiencing homelessness. Skills and Capabilities Support for and an understanding of the mission and purpose of The Salvation Army in Canada. Possess information and knowledge around homelessness, mental health, addictions, substance abuse and harm reduction. Possess superior knowledge of community resources. Possess thorough knowledge and understanding of a commitment to anti-racism, anti-oppression, and equity. Possess excellent problem solving, communication, organizational and computer skills. Possess excellent interpersonal and time management skills. Possess a positive attitude and works well with other team members. Self-motivated and disciplined Good communication skills (verbal and written) in English Must be able to lift a minimum of 20 pounds. To maintain confidentiality in all matters pertaining to clientele and staff of The Salvation Army. Valid Driver’s License and must provide clean driver’s abstract. Compensation: The target hiring range for this position is $21.83 to $27.29 with a maximum of $32.75. Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
