Job Description: Reporting to: Director of Care Specific Duties and Responsibilities: Understands, supports, and implements the Mission, Vision, Values of the organization. Provides clinical oversite to the care team, residents, and their families in accordance with organizational policies, procedures, and programs incompliance with BCCNM standards of practice. Provides on-going resident assessment, formulation, implementation, and evaluation of care plans in coordination with the Care Team. Responsible to work within Point Click Care, EMAR, Pathways , and Excelleris Collaborates with a multidisciplinary care team of physicians, therapists, and other care professionals. Ensures that the administration of medications is in accordance to established policies and procedures including direct dialogue with physician, pharmacist, resident, and family Performs special procedures as needed; including but not restricted to catheterization, tubes, suctioning, super pubic catheters, etc. Ensures medical records are inclusive of clinical documentation. Coordinates referrals and communication with community partners including Wound Clinician, Pharmacist, Fraser Health Authority, etc. Participates in unit and staff meetings, medication reviews, care conferences, as well as any meeting with facility community partners ( pharmacist, wound clinician, etc.). Oversees the admission process including the liaison with transferring facilities, involvement with family. Performs and documents InterRai Resident Assessments as needed. Promotes and maintains infection control program. Is responsible for implementing infection control precautions including the collection of samples as well as notification of outbreak to Public Health. Responsible to collaborate with BCCDC and MHO. Reports risks, hazards, near misses, unsafe situations, incidents, and accidents to DOC. Supports teamwork in a collaborative manner, is knowledgeable about the roles of other multidisciplinary team members and is accountable for own decisions and actions. Provides input for staff performance. Promotes effective communication with residents, families, visitors, physicians, staff at all levels and other health-care agencies and facilitates inter-shift, inter-disciplinary and inter-departmental collaboration. Participates in continuous quality improvement, safety, accreditation programs, and team meetings as required. Serves as a role model and clinical resource for Nursing staff by demonstrating clinical expertise, providing hands on direct clinical guidance, and sharing expertise. Maintains current knowledge and certification applicable to job qualifications. Is knowledgeable about and adheres to all policies. Assumes all responsibilities of unit nurse when short staffed or needed. Assumes on-call responsibilities, as needed. Assumes the leadership role in emergency situations. Follows the fire/disaster responsibilities. Acts as the First Aid Attendant for facility Performs other related duties as required. Job Qualifications: RN Diploma or Baccalaureate Degree from an approved school of nursing with current practicing registration with the British Columbia College Nurses and Midwives (BCCNM). Gerontology certificate preferred. Current CPR certification Level C. WHMIS (Workplace Hazardous Materials Information System) Ability and willingness to undergo applicable screening and background checks successfully that are satisfactory to The Salvation Army, in its sole discretion (i.e., The Salvation Army Abuse Registry) and provide an original copy of a Background Check screening. Experience: Previous experience in geriatrics or long-term care is an asset. Physical: Must be well groomed, maintain good physical, and mental health, and be fit to perform above duties which include standing, bending, lifting, etc. Special: Must demonstrate concern and interest in providing quality care to the elderly. Must be able to work in a collaborative manner with residents, families, and teams in a manner which demonstrates professionalism, empathy, and patience. Must demonstrate ability to communicate effectively. Must maintain role model of good ethics and job performance. NOTE: Not all incumbents perform all of the duties described above but perform a similar range of duties.
