The opportunity:
As a Youth In Transition Case Manager you will develop and deliver programming to Youth in Simcoe County (Youth Justice Program, Youth CAS Program, Paid Employment Program, Entrepreneurship Program, Newcomers). Areas of service delivery may include stress and anger management, positive communication skills, self-confidence building, employment readiness, life skills, independent living, etc. Provide outreach, follow-up support and assistance to clients. Develop and facilitate group programming on topics identified as needed to assist these youth. Conduct outreach and liaison in the community to youth and youth serving agencies.
NOTE: This is a one year contract.
In this role, you will:
Demonstrate strong support and commitment to the mission and core values of the YMCA of Simcoe/Muskoka and the YMCA in Canada.
Demonstrate strong support and commitment to serving the membership, facility users and all program participants of the YMCA of Simcoe/Muskoka.
Maintain high quality program in accordance with YMCA standards and best practices, participant needs and Association strategic directions.
Monitor and maintain high participant retention through the delivery of exceptional programs and services.
Build collaborative working relationships with funding partners, mentors, and other community partners through ongoing reports, communication and visits.
Prepare and coordinate all monthly reports for the program, including court reports if applicable.
Actively participate with the supervisor and community partners (probation officer, case manager, etc.) to assess the youth needs and develop a case plan based on the individual’s needs and mutually agreed upon goals with the client.
Provide individual and/or group counselling for youth.
Provide counselling and advocacy services to youth at various locations throughout the service region as assigned by the supervisor.
Assist at-risk clients to determine appropriate service delivery or client vocational limits.
Prepare clients for success through facilitation of individual counselling or group workshops
Effectively manage a caseload, completing case notes following each contact, setting appropriate job priorities consistent with the various government ministries and department.
Consult with social services and community agencies to ensure effective coordination and provision of services.
Participate in case conferences or consultation where appropriate.
Ensure all administrative duties, reporting, and file management, are completed in an accurate and timely manner.
Exercise sound judgment in making decisions which affect the welfare of youth clients.
Work with multi-barried youth, including those in crisis, on a regular basis, sometimes in isolated situations.
Adherence to all provincial and federal legislation relevant to job description.
You bring:
Post-Secondary Education in Social Services, Child and Youth Worker or combination of education and work experience
Minimum 2 years’ related work experience
Counselling experience both one-on-one and in a group setting
Knowledge and understanding of community systems and resources
Experience working with youth facing barriers
Strong verbal and written communication skills, including letters, memos, reports and presentations
Life Skills Coaching certificate an asset
Leadership management an asset
Healthy Adolescent Development training an asset
Must have a valid driver’s license and access to a reliable vehicle
Willingness to participate in mandated training to be completed before or shortly after commencement of work including:

PCI Compliance
WHMIS
Child Safeguarding Orientation
Willingness to be able to work flexible hours based on client needs
