360°kids has been a leader in the delivery of innovative support services for over 30 years as we work to improve the lives of homeless and at-risk youth and families in York Region. We are a vitally important community agency serving all of York Region. At 360°kids, we provide a unique continuum of care approach with a range of programs and services that are essential to the health and well-being of youth and that are proven strategies for helping to keep youth off the streets by providing safe housing options.
The Youth Worker will provide direct, in-person case coordination and management to youth enrolled in the BYHP and Youth Leaving Care Transitional Housing Programs. Working collaboratively with youth and community partners, the Youth Worker will support participants in developing and implementing personal action plans that align with program outcomes, with a strong focus on mentorship, life skills, financial capacity, connection to community, advocacy, health and wellbeing and housing stability.
The youth worker will take a proactive role in developing and facilitating structured, targeted programs designed to meet the specific needs of youth of these historically underserved and marginalized groups. Additionally, the youth worker will actively cultivate and maintain strategic partnerships with community organizations, schools, and service providers to collaboratively address gaps and barriers that these youth have traditionally experienced, thereby fostering inclusive opportunities for growth, learning, and empowerment.
Key Responsibilities
Deliver case coordination and management for youth aged 16–26, in alignment with program deliverables associated with each home/program.
Facilitate predictable weekly visits to each program location to deliver programming and foster/maintain positive rapport with youth
Mediate challenges between suitemates as they arise to promote problem-solving, communication, collaboration and sustainable housing arrangements.
Conduct and document monthly health and safety home inspections and liaise with the Housing Coordinator to ensure regular maintenance of the facilities.
Report regularly to the Program Manager or CAS worker, regarding youth progress, and the Housing Coordinator for any maintenance related or urgent facility concerns
Conduct or assist with planning and facilitating both mandatory and optional life skills workshops and activities.
Support the development, coordination of individualized youth action plans; monitor and document progress.
Documenting of client interactions in agency database in keeping with PHIPA and other privacy laws.
Mitigate, and track client and community member complaints by maintaining standards and to minimize reputational risk to the organization.
Provide informal counseling, mentorship, and role modeling tailored to the unique needs of each youth.
Advocate for youth in collaboration with external service providers when needed.
Encourage positive youth development through consistent coaching, role modeling and support.
Help youth access and remain connected to relevant services, including mental health, medical, educational, mentorship, entrepreneurial, employment resources.
Track and coordinate collection of monthly occupancy payments from participants and ensure timely transfer to the Finance department.
Maintain consistent communication with the Program Manager, CAS designate, Housing Coordinator and Community Partners.
Develop partnerships to support community engagement and program delivery.
Foster and maintain open communication with community partners that support youth in achieving their goals
Participates in regular supervision meetings and performance appraisal meetings on a semi-annual basis
Education and Skills
Degree or diploma in Social Work, Child and Youth Work, or a related field.
Minimum 2 years’ experience working with marginalized or at-risk youth and/or Youth in Care, preferably within a housing or transitional living context.
Current First Aid/CPR certification and Current Broad Record Check (criminal background check).
Must have a valid driver’s license and access to a vehicle
Strong understanding of systemic issues impacting Black youth, and youth in care including equity, housing insecurity, and barriers to wellness.
Knowledge of Ready, Set, Go Program for youth leaving the care of child welfare.
Experience in case management, mediation, and collaborative service planning.
Ability to build trust and maintain professional relationships with youth, community partners, and other relevant stakeholders.
Strong problem-solving, communication, and documentation skills.
Knowledge of relevant community resources, supports, and referral processes.
Ability to work independently and as part of a team.
Shift timing: Monday-Friday; 10-6 with flexibility
