GENERAL ACCOUNTABILITY:
The Communicable Disease Control Coordinator is accountable for direct supervision of communicable disease public health nurses. They are responsible for the coordination of the public health activities to control and prevent communicable diseases, including surveillance and reporting responsibilities, as outlined in The Public Health Act, 1994 and The Disease Control Regulations and Saskatchewan Health directives and policies and other applicable federal legislation. They will coordinate the prevention, planning, implementation and evaluation of Communicable Disease Control (CDC) programs within the Saskatchewan Health Authority and their designated areas. The CDC Coordinator’s scope of practice requires a broad knowledge base and autonomous decision-making skills, with a specialized focus on implementation of provincial standards related to Communicable Disease Control. The coordinator role provides program orientation, ongoing in-servicing and support to designated CDC response staff, including Public Health Nurses within an interdisciplinary service delivery model. CDC. This position is also responsible for auditing program standards for staff adherence, as well as CDC reporting required by the Ministry.
ORGANIZATIONAL STRUCTURE:
The Communicable Disease Control Coordinator works under the direction of the designated Clinical Integration Manager, in collaboration the network Primary Health Care (PHC) Manager(s) and the network PHC Director(s).
KNOWLEDGE, SKILLS AND ABILITIES:
Education: Bachelor’s degree in Nursing with course credits in community health and public health nursing required.
Experience: Candidates with a minimum of 5 years recent relevant experience.
Other: Registration or eligibility for registration with the College of Registered Nurses of Saskatchewan (CRNS)
Knowledge and ability to apply community health nursing theory and principles.
Ability to apply intermediate computer skills and applications as required.
Possession of a valid Class 5 driver’s license.
Skills in leadership (i.e. Lean principles, LEADS and other Leadership Training / experience may be considered)
JOB KNOWLEDGE:
• In-depth knowledge of The Public Health Act, the Disease Control Regulations and relevant Saskatchewan Health directives.
• Current and thorough understanding of disease agents, disease control issues and the epidemiology of communicable diseases as well as relevant legislation and regulations, policies and guidelines. Utilizes epidemiological models to explain disease patterns in community health.
• Knowledge of community development and population health promotion concepts.
• Knowledge of research and statistical collection concepts.
• Skills in oral and written communication.
• Skills in program planning and evaluation specifically the development of standards of care/guidelines/policies and procedures pertaining to Communicable Disease Control.

PRIMARY RESPONSIBILITIES:
The Communicable Disease Control Coordinator:
• Provides clinical supervision to Public Health Nurses that primarily work within the CDC stream. They also provide clinical consultation to network leaders and other public health staff.
• Manages the orientation of designated CDC staff.
• Advises the appropriate PHC manager(s) in matters pertaining to program management, staffing, and resource requirements;
• Consults with designated CDC Public Health Nurses regarding workload and public health priorities (e.g. client care; program implementation, legislated responsibilities)
• Collaborates with the appropriate PHC manager for both probationary and regular performance evaluations of designated CDC staff.
• Coordination of community based outbreak investigations in partnership with the area Medical Health Officer, Senior Public Health Inspector, network directors, Infection Control, physicians, and other public health field staff (PHNs and PHIs) where appropriate. Liaison with other Provincial and Federal agencies depending on the magnitude of the outbreak. Assist with the development of questionnaires, data collection, analysis and preparation of reports.
• Coordination of the follow up of animal bites/exposures requiring post-exposure prophylaxis for rabies. This includes liaison with Public Health Inspectors, physicians, other facility-based health professionals, CFIA veterinarians and local veterinarians.
• Works in collaboration with Directors and Managers of PHC / Public Health as well as the Medical Health Officers in development and management of CDC programs to ensure the quality of program delivery.
• In partnership with the designated managers, provides evaluation of applicable CDC related services; contributes to planning, implementation and evaluation of policies. Advises designated manager regarding the need for program development and revisions;
• Ensures designated CDC response, including Public Health Nursing programs and services meet the needs of the community; (e.g. efficient, timely, equitable, responsive);
• Formulates, implements, and evaluates standards for public health nursing and interdisciplinary practices and communicates them to staff and, as necessary, to others;
• Participates in accreditation process and participates on quality improvement teams;
• Collaborates with human service partners in providing CDC initiatives, including and prevention education programs (i.e.: Education, Social Services, Justice);
• Ensures that client, employee and employer confidentiality and anonymity is maintained and consistent with Saskatchewan Health Authority policy and other applicable privacy legislation (e.g. HIPA).
• Coordinates processes for achieving and maintaining competencies of designated CDC response staff, including Public Health Nurses
• Prepares designated CDC staff to deliver CDC related services through supporting initial and ongoing competencies through orientation, mentorship and ongoing in servicing.
• On an ongoing basis, ensures designated CDC Public Health Nurses demonstrate the knowledge and skills to deliver the current CDC programs;
• Participates in designated CDC Public Health Nurses orientation and reorientation in relation to program delivery
• Plans and provides ongoing education and development of the knowledge and skills required for effective and safe delivery of Communicable Disease Control programs.
• Provides timely updates on new and emerging communicate diseases to entire public health staffing unit.
PHYSICAL DEMANDS:
The physical exertion in this position is moderate and generally involves lifting, carrying, bending, driving, and walking/standing for more than 30 minutes at a time. Extended time spent doing key entry functions.
