Program Overview – Supportive Housing, Community Care
The Supportive Housing Program provides housing for seniors who have been
street-involved, and who may be experiencing mental health and/or addictions
issues. The program provides onsite psychosocial, primary care supports and
services including intensive case management; harm reduction and addiction
support; therapeutic and supportive counselling; 24-hour staffing; assistance
with activities of daily living; nursing services; life skills development;
social and recreational activities; volunteer opportunities; and follow up
support.
What You Will Do
- Provide reception services to staff, and residents living at in supportive
housing program.
- Ensure the safety of the building and its occupants after regular business
hours.
- Monitor activities in the common areas and the outside perimeter of the
building through the security cameras/visual inspections and responds
appropriately.
- Implement crisis response strategies, including de-escalation, support,
debriefing, etc. when necessary.
- Complete hourly rounds on the participant floors.
- Provide after-hours supportive counselling to residents when required.
- Re-direct any visitors and responds to issues appropriately.
- Perform lock up and alarm duties in accordance to established procedures.
- Respond to the 24-hour resident personal alarm system according to
appropriate procedures.
- Understand how to operate, trouble shoot and contact the 24-hour response
team for the emergency response system as necessary.
- Carry out appropriate First Aid responses, when necessary.
- Work effectively with other team members, including on site Personal Support
Workers, to address resident and building issues.
- Complete a detailed participant activity log & shift exchange, reporting any
incidents, occurrences, health and safety risks, status of resident
community, resident interpersonal conflict.
- Understand risk management procedures and responds professionally and
appropriately. This includes, but is not limited to responding to all
emergency situations, such as managing a resident emergency (e.g. illness,
fire in the unit or building), or emergency that affects building operations
(e.g. elevator breakdown, power failure).
- This position will include some evening and weekend shifts monthly and is
subject to exposure to second-hand smoke, odors, pests and clients’ potential
use of inappropriate language or swearing.

- Clients may exhibit responsive behaviours including profanities and
expletives.
- Able to lift 40lbs
What You Bring to the Team
- 2 Year diploma in social services or certification in security.
- 3+ years of experience working with older adults and/or seniors with mental
health, addictions issues, and street involvement.

- Demonstrated ability and experience dealing with aggressive behaviours and
effective de-escalation and boundary setting skills.
- Demonstrated ability in written, oral, and interpersonal communication
skills.
- Demonstrated ability to work independently within established agency policies
and procedures.
- Demonstrated ability in conflict resolution and crisis intervention skills.
- Demonstrated use of good judgment.
- Experienced team player.
- Demonstrated competency with using computers, databases, and web-based
applications.
- Knowledge of geriatric and aging issues.
- Knowledge of harm reduction approach.
- Knowledge of mental health act.
- Knowledge of community resources and various systems in Toronto.
