The Registered Practical Nurse (RPN) will provide high quality patient care within the pediatric diabetes and endocrine clinic, according to the professional standards set by the College of Nurses of Ontario, functioning as an active member of the multidisciplinary health care team using a client centered approach. The RPN is responsible for assessing care needs, implementing the health care plan and providing education and follow-up to patients and caregivers. The RPN will work closely with interdisciplinary team members within the pediatric diabetes and endocrine clinic case management, participating in patient education initiatives, understanding and supporting clinic related research projects, performs other duties as assigned.
Current Certificate of Registration from the College of Nurses of Ontario as a Registered Practical Nurse
Current Basic Cardiac Life Support (BCLS) certification
Recent experience, within the last 3 years, in the care for children preferably in a tertiary care outpatient setting
Demonstrated knowledge and understanding of the approach to pediatric assessment, care planning, intervention and follow-up
Previous experience with clinic coordination or patient navigation preferred
Previous experience with managing clinic flow, obtaining vital signs and patient data, communicating results to the interdisciplinary team, and coordinating follow-up
Excellent teamwork, communication, problem-solving, and organizational skills
Demonstrated knowledge, skill and ability in health teaching with pediatric patients and their families individually and in a group setting
Demonstrated ability to communicate with children and their families under stressful conditions
Proficiency with computer applications
Knowledge, skill and ability to provide health teaching for pediatric patients and their families, specific to the needs of the clinic population
Demonstrated knowledge of motivational interviewing
Demonstrated skills in psychosocial assessment
Ability to adapt to rapid change in patient condition workload status
Demonstrated leadership abilities and the ability to organize and coordinate patient and family-centered care with other members of the healthcare team
Demonstrated ongoing professional development
