As a result of this growth, we are currently looking for someone to join our dynamic Management Team in the role of Clinical Practice Lead. As a member of the management team, you will work closely with the Director, along with other key internal and external partners, in growing the nursing program within the region.
The Clinical Practice Lead will be accountable for the administration, organization and management of Community Nursing teams, ensuring the achievement of quality care standards that are consistent with the core values of the organization including patient-centeredness, excellence, partnership, and accountability. The Nurse Manager - Clinical Practice Lead will also be responsible for operational and leadership functions including meeting performance targets, onboarding and orientation of new nurses, and continued mentorship, teaching, and performance management of teams. These responsibilities shall be carried out in accordance with company policies, procedures, mission, vision and applicable legislation. During the growth phase of the program, the Nurse Manager - Clinical Practice Lead will operate in a working manager capacity. This will include performing up to 20 nursing visits per week while also supporting current nurses and growing the nursing team to sustainably support the community.

This is a Full-Time position. Hours of work will be Monday-Friday as well as participation in some On-Call work, which includes evenings and weekends on a rotational basis.

What The Role Involves
Our leadership team plays a critical role in providing high quality patient care, mentoring our health care teams and inspiring others.
Providing professional and clinical leadership to direct report nursing staff (both visiting and shift nursing)
Acting as an ambassador for CarePartners, consistently representing the organization with professionalism, tact, and in alignment with its core values
As required, acting as a liaison between staff and the College of Nurses of Ontario regarding standards of practice etc.
Working closely with others both internally and externally, as well as independently
Building strong teams that consistently deliver excellent care that is patient- and caregiver-centered, grounded in evidence-based clinical practice, and in a fiscally responsible manner
Translating strategic goals into practical actions that are sustainable
Conducting objective and constructive performance reviews including clinical reviews as scheduled
Participation in the managers' on 24 hour/7day call rotation for the designated geographic area by carrying a pager and responding to pages promptly and appropriately
Identifying, monitoring and problem solving around clinical and operational issues impacting the patient and caregiver experience; takes a quality lens to identify and implement solutions that are evidence-based, innovative and sustainable for the program
Being actively involved with all safety and quality issues in the branch and agency
Actively promoting and participating in the agency's Continuous Quality Improvement Program and Risk Management Program
In collaboration with the Director, is responsible for both short-and long range planning for the branch
In collaboration with the Director, looking for opportunities to support integrated care working closely with internal team members (i.e. Personal Support Managers) and external partners (i.e. local Ontario Health Teams) to drive a seamless experience for patients and caregivers
Demonstrating flexibility and ability to carry forward long term projects as well as deal with daily issues and crises
Performing other related work as assigned
What You Bring
Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
3 - 5 years of job-related experience or an equivalent combination of education and experience
Registration with the College of Nurses of Ontario (CNO)
Experience in a community healthcare environment considered an asset
Experience in a formal leadership role considered an asset
Proven computer skills, including email, word processing, work scheduling systems
Thorough knowledge of case management skills and nursing processes and practices
Vehicle is required and some regular travel is required
Leadership and people management skills are an asset
Excellent English verbal and written communication skills
French language skills are an asset
