We are Somerset West Community Health Centre (SWCHC), a non-profit, community-governed organization that serves over 18,000 clients annually. Established in 1978, SWCHC provides a wide range of health care services such as primary health care, health promotion, community development, mental health, and harm reduction. What the Program Entails
SWCHC is well known for its excellent Community Health Centre (CHC) model and its professional health services team, but it has also become a leader in offering a wide range of community-building activities and advocacy programs. Our team of doctors, nurses, dietitians and more offer a range of medical services.
What You’ll Do
Reporting to the Director of Family and Community Services, the Registered Nurse (RN) works as part of an interdisciplinary team to focus on prevention, early intervention, and equity in access to care within primary care, mental health, and teams. The RN collaborates with interdisciplinary teams, community organizations, and local partners to improve population health outcomes and address the social determinants of health, particularly among Black, racialized, underserved, and newcomer populations. The RN will work alongside the Manager of Equity and Integration for Black Health and play an active role in health promotion during wellness and mobile clinics. They will also conduct community outreach events in collaboration with internal SWCHC teams and external partners and support the evaluation of our health programs for this diverse population. Functioning within a robust team and collaborative environment, the RN will help advance program goals and enhance health outcomes within the community
What We’re Looking For
◈ Life and Work Experience
Three to five years’ experience in community nursing working with diverse populations
Previous clinic experience an asset
Previous experience using an EMR, preferably Practice Solutions Suite (PSS)
Experience providing routine immunizations for all ages
◈ Your Education Path
Current license with the College of Nurses of Ontario (RN)
Current CPR (preferred)
◈ Required Language
Fluency in English required
Working knowledge of French or other languages an asset

Knowledge of primary health care and the social determinants of health
Able to work effectively in an inter-professional environment
Knowledge and awareness related to cultural competence
Experience working with disadvantaged communities
Knowledge of nursing care related to Primary Care/Community Health/Chronic Disease Management
Your Day-to-Day at a Glance
As our Registered Nurse, your responsibilities include, but are not limited to:
Plan, implement and support the evaluation of health promotion mobile clinics
Organize and participate in community health mobile clinics and events focused on preventive health (e.g., cancer screening, diabetes monitoring and blood pressure checks)
Perform nursing assessments at health and wellness mobile clinics
Conduct one-on-one and group health education sessions with clients across teams
Support referrals and navigation to appropriate health and social services
Collect, analyze, and report data to support program evaluation and funder requirements.
Work collaboratively with nurse practitioners, physicians, social workers, dietitians, and mental health professionals to provide holistic care.
Participate in team meetings, training sessions, and quality improvement initiatives
