Position Summary:
Be responsible for the delivery of patient-centered clinical nutrition therapy to Renal and medicine inpatients
Provide quality nutrition therapy in relation to the patient's diagnosis, treatment, age, and nutrition status in a fast passed inpatient setting
Required to work day shifts and cover weekends in the ICU
Provide screening and assessment; contribute to care plan development; establish goal-oriented treatment interventions; conduct patient/family education; and provide support to address nutritional needs
Educate patients and family members as well as THP staff and professional staff on therapeutic diets and nutrition therapy as needed.
Act as a coach/mentor/consultant using sound critical thinking skills to achieve patient outcomes and to facilitate team function
Act as a preceptor for dietetic interns and coach/mentor other students as appropriate
Participate in interprofessional patient care teams, hospital/regional committees as required, and provide education to medical and nursing students
Complete daily workload statistics
Qualifications:
Degree in Human Nutrition and Dietetics, and a Dietetic Internship, or Master's or Diploma practicum program required
Current registration with the College of Dietitians of Ontario required
Completion of Dietitians of Canada Critical Care Nutrition Therapy course preferred
Established clinical skills in nutritional management of medical and renal patients
Experienced in the management of enteral and parenteral nutrition support
Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively within a high-paced team environment
Effective interpersonal, organizational and communication skills
Demonstrated ability to utilize practice-based research to ensure excellence in the provision of nutritional care.
Proficiency in MS Teams and hospital information system – Epic (preferred).
At least two years’ recent experience with established clinical skills in an inpatient acute care setting
