Position: Client Access Worker & Care Coordinator
Component/Team: Client Access Team/AMHS
Status: 1 Year Contract, 0.5 FTE Client Access Worker and 0.5 FTE Care Coordinator (for a combined 35 hours per week)
Salary Scale: $56,839 - $68,468 annualized salary plus benefits
Start Date: Immediately

Position Description:
Client Access Worker
The Client Access Worker’s main role is to facilitate groups such as SMART Recovery meetings, SMART Recovery Family and Friends meetings and other psycho-educational groups related to substance use and/or family support. The Client Access Worker also plays a role in engaging, welcoming, assessing the needs of, and providing direct services to clients and community members. The Client Access Worker provides information, intake, preliminary assessment and referral services to ensure seamless service provision.
The main activity for this role is group facilitation. Other activities may include intake and assessment and basic needs assistance.
Care Coordinator
The Care Coordinator, AMHS plays a vital role in supporting individuals experiencing mental health and/or substance use health, and/or problem gambling challenges to access, engage, and remain connected with appropriate treatment, recovery, and support services. Working within a multidisciplinary team, the Care Coordinator conducts comprehensive assessments, develops and monitors care plans, and coordinates services to promote holistic, person-centred recovery and wellbeing.
This role ensures continuity of care across providers, fosters collaboration among diverse clients, families, and community supports. The Care Coordinator supports clients who require stabilization or who face barriers—such as unmet social determinants of health, including housing instability, limited income support, or lack of access to primary care—that affect treatment engagement and their ability to access and maintain ongoing care.
The Care Coordinator provides guidance, advocacy, and system navigation, maintaining strong partnerships and up-to-date knowledge of community programs and resources.
Reporting Relationship:
The Client Access Worker is directly accountable to the Client Access Team Lead, through the Team Lead to the Director of Community and Social Services and to the Executive Director.
The Care Coordinator is directly accountable through the Addictions and Mental Health Services Team Lead to the Director of Community and Social Services and to the Executive Director.
Skills, Education and Experience:
Requirements for this position include:
Education and Language
A Baccalaureate degree in Social Sciences or a related discipline
Be a member in good standing of a college or order such as the College of Registered Social Workers and Social Service Workers
Under the Centre’s designation to provide French Language Services:
French (oral expression): Advanced
French (oral comprehension): Advanced +
French (reading comprehension): Intermediate +
Fluency in English, both oral and written
Professional Experience
Experience facilitating groups such as SMART Recovery
Two to four years’ experience providing case management, system navigation and / or care coordination
Experience working with individuals who experience concurrent disorders within a harm reduction framework
Experience conducting intake and preliminary psychosocial, substance use, problem gambling and mental health assessments
Experience with motivational interviewing or other engagement strategies
Experience working with a broad range of populations, including: at risk youth, street-involved people, people with mental health challenges and/or problematic substance use and their loved ones, multi-cultural communities and people experiencing significant barriers to access
ASIST, Non-violent Crisis Intervention (NVCI), Motivational Interviewing, CBT and SMART Recovery training an asset
Knowledge, Skills and Abilities
Ability to facilitate education groups of up to 25 participants
Knowledge, understanding and sensitivity to substance use, problem gambling, mental health issues and concurrent disorders
Ability to work within a harm reduction framework
Knowledge of community resources in Ottawa and addiction treatment programs and services
Knowledge and experience of the various approaches to substance use/concurrent disorders
Strong interpersonal, organizational and problem-solving skills
Strong intake, assessment, triage and crisis intervention skills
Knowledge of the wide-ranging impact of determinants of health
Ability to incorporate and strengthen collaborative and interdisciplinary teamwork
Ability to de-escalate crisis situations in a therapeutic manner
Effective communication skills
Ability to work under stress with multiple demands and tasks
Respect and valuing of the diversity of communities and individuals
Ability to work independently
Ability to ‘think outside the box’; creative thinking

Hours of work will be developed in collaboration with the successful candidate to ensure a healthy work-life balance. A schedule of work will be based on the Centre’s hours of operation and program requirements and may include some evenings.
