What You'll Do At Gaudenzia
• Be responsible for level of care (LOC) assessments for individuals seeking
treatment, as identified by program requirements and in accordance with state
regulations.
• Complete required evaluations and assessments, including screening, intake,
clinical assessments, American Society of Addiction Medicine (ASAM) and
biopsychosocial histories.
• Complete face-to-face, virtual, and/or telephonic interviews with individuals
presenting for LOC assessment and/or admission to the appropriate program.
• Accurately document assessment/intake records, records of referral, and
funding source approvals/authorizations, and diagnosis within the electronic
health record’s (EHR) system.
• Document all case management and clinical services required by the Single
County Authority (SCA), into the Pennsylvania Web Infrastructure for Treatment
Services (PA WITS) system and within the company’s electronic health record’s
system.
• Maintain accurate electronic records on each client and ensure the timely
completion of tasks, based on current policy and procedure and the appropriate
funding source requirements.
• Maintain proficiency in assessment, intake and level of care protocols,
utilizing the ASAM criteria.
• Conduct admission funding reviews with relevant payor(s) and assist with
completion of medical assistance (MA) applications, as needed.
• Participate in funding source reviews and make recommendations for treatment
and lengths of stay as necessary for clients admitted to any of the Gaudenzia
facilities.
• Coordinate with agency and community resources to ensure access to the most
accurate and up-to-date interim services programs.
• Maintain accurate information for follow-up in the PA WITS system, along with
the company’s electronic health records system.
• Establish and maintain positive relationships with funding, referral, and
regulatory agencies.
• Follow up with clients after discharge based on regulatory guidelines and
document appropriately in PA WITS and the agency’s EHR system.
Pennsylvania Position Requirements:
Associate’s Degree with two years’ clinical experience
Bachelor’s degree with one-year clinical experience
Acceptable degrees include (but are not limited to): Social Work, Psychology,
Addiction, Counseling, Health/Human Services, Criminal Justice, Education,
Health Administration, Physical Therapy, and more.
