Neighbourly Pharmacy is looking for an Infusion Clinic Coordinator to join our team at Sandstones Pharmacies Glenmore Landing in Calgary, AB.
Coordinating patient care between physician offices, pharmacies, and the infusion clinic using multiple software programs.
Responsibilities:
Ensure timely completion of patient work-up to optimize patient starts and therapy transitions
Main point of contact for all key stakeholders
Schedule and coordinate infusion appointments based on treatment plans
Communicate with patients regarding appointments, instructions, and follow-ups
Complete patient enrollment for PSP’s
Coordinate services with patients and physician offices
Monitor patient progress and report concerns to the appropriate care team members
Track and follow up on pending approvals to avoid delays in treatment
Coordinate with billing teams to ensure appropriate coding and reimbursement
Provide compassionate and effective communication with patients and caregivers
Liaison with pharmacy and RN staff
Facilitate and complete third party billing
Qualifications:
Previous experience with AHS referral coordination/biologics coordination/specialist referrals preferred
Knowledge of infusion therapies, medication names, and treatment protocols is a plus
Offer unique services that will compel MDs to actively refer patients
Ability to build relationships with key stakeholders
Innovate, lead and help us expand specialty services
Provide patients with exceptional service
Excellent organizational and multitasking abilities
Proficiency in electronic health records (EHR/EMR), Microsoft Office Suite
Ability to problem solve and work independently
