Job Description: POSITION PURPOSE SUMMARY: The Transition Specialist II delivers services through a wide range of community-based ministry programs in accordance with Salvation Army Mission, values and standards. ACCOUNTABILITIES: Program Development / Administrative Duties: · Assist with development, implementation and facilitation of community-based programs to make a positive impact on community · Promote the program internally and externally as required, fostering positive relationships with community partners and agencies · Continual research of community services and resources to meet client requirements · Maintain accurate and current file management · Participate in program strategy development · Various administrative duties which include documentation, tracking statistics and assisting other program staff as needed · Support, respect and give appropriate feedback to other team members · May be required to support the Program Manager in providing oversight to Transition Specialist Case Management: · Interview clients, assess clients’ needs and make appropriate referrals to internal and external programs/services/resources · Support and collaborate with clients in establishing goals, monitor progress, and prepare independent living plans/care plans, encourage problem solving and decision making · Demonstrate compassion while modeling appropriate behavior and appropriately intervening in crisis situations · Align with and support the mission and vision of The Salvation Army, linking individuals to Corps (church) family and programs where possible · Perform all other duties as required CRITICAL RELATIONSHIP MANAGEMENT Internal: · Various ad hoc Committees as assigned MANAGERIAL RESPONSIBILITY: · This position reports directly to the Program Manager · This position has no direct reports. FINANCIAL AND MATERIALS MANAGEMENT: · Confidential Client Files and Reports · Maintains a pool of resources with other community services for easy accessibility by the residents. WORKING CONDITIONS: · Working environment is typically in the office or at work sites within residential settings in generally agreeable conditions · Occasional travel and some scheduled overtime is associated with this position • The incumbent’s work environment is typically at work sites within residential settings, and may experience the following disagreeable conditions: therefore, will need to follow or use universal precautions: o Dealing with angry and abusive people, verbal abuse o Exposure to bodily fluids, bodily waste, and infectious diseases o Exposure to drugs, alcohol, and solvents o Physical attacks by others o Exposure to toxic chemicals o Sharp objects (i.e., needle sticks, knives, and weapons) o Other dangerous situations o Immunization for Hepatitis B and vaccination against Tuberculosis The above responsibilities must be discharged in accordance with The Salvation Army’s Mission Statement, in a professional manner, exemplifying Christian standards of conduct. EDUCATION AND EXPERIENCE QUALIFICATIONS: Education, Qualifications and Certifications: · Post-Secondary degree/diploma in Social Work or other Human Services field · Must be a registered Social Worker with Alberta College of Social Workers · Valid Driver’s License and clear Driver’s Abstract · Current certification in First Aid with CPR · Current certification in Non-Violent Crisis Intervention Experience and Skilled Knowledge Requirements: · Previous experience in social services/mental health setting · Minimum 3 years of experience working with inner city agencies/services and clients · Experience with case management is strongly preferred · Display competence in developing and maintaining appropriate client relationships and interagency relations · Extensive knowledge of community services, agencies and supports · Ability to enter data and perform statistical analysis efficiently and accurately · Experience with community or program development considered an asset · Experience with public speaking or group facilitation considered an asset · Respect and understanding of The Salvation Army - its mission, culture, and values Skills and Capabilities: · Competent to develop and maintain client relations and interagency relations. · Excellent organizational, interpersonal and communication skills (written and oral) · A team player; ability to work in a compassionate, respectful, and non-judgmental manner · Proficient in Microsoft Office applications This is a temporary Full-time fixed term employment for maternity coverage. Weekly scheduled hours: 40 hours Target hiring range for this position is $31.32 per hour at Step 1 to $33.29 per hour at Step 4 Compensation: The target hiring range for this position is $31.32 to $32.31 with a maximum of $33.29. Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
