About the Role:
The Case Coordinator plays a key role in supporting individuals experiencing housing instability by delivering client centered, culturally informed services within the Thunder Bay Transitional Housing Program. This position is responsible for the delivery of program goals, maintaining a caseload of participants, and providing housing-based case management focused on long term stability and self sufficiency.
Core duties include developing and implementing individualized service plans, facilitating referrals to internal and external supports (e.g., financial assistance, mental health services, legal aid), and supporting participants through crisis intervention and life skills development. The Case Coordinator also monitors compliance with program policies, delivers workshops on tenancy and life skills, and collaborates with Elders, Knowledge Keepers, and service providers to ensure culturally appropriate programming.
Administrative responsibilities include maintaining accurate case files and documentation, contributing to program evaluation and promotion, and ensuring services align with funding agreements and community engagement goals. The role also involves active participation in community and cultural events to support participant integration and understanding of available resources.
Strong relationship-building, crisis management, cultural competency, and communication skills are essential for success in this position, as is a commitment to confidentiality, client empowerment, and social inclusion.
What you will be doing:
Delivery of associated program goals including delivery of services and supporting participants within Thunder Bay Transitional Housing Program
Maintain caseload of clients (aka participants) and provide housing-based case management as required
Build strong and effective rapport with participants
Monitor, enforce, and support program policies and participant agreements as required
Plan and execute a client service plan, including referrals to internal programs and other agencies that provide financial assistance, legal aid, mental health and addiction services, medical treatment and other services as required
Develop and deliver educational materials/workshops on tenant rights and responsibilities, how to promote a successful tenancy, basic life skills and other rental related matters to assist participants in developing skills to deal with and resolve identified challenges specific to maintaining stable housing
Plan, participate and coordinate with Elders, Knowledge Keepers, internal MNO programs, and external services providers to facilitate workshops for participants within the Thunder Bay Transitional Housing Program in areas related to Indigenous culture, life skills, mental health, physical health, tenant rights and responsibilities, education, employment, and any other identified areas of need to help promote long term housing stability.
Provide crisis intervention to participants and deescalate situations as required
Maintain open lines of communication throughout shift with other Thunder Bay Transitional Housing Program staff, including those from other agencies and external contractors (e.g. concierge/security services).
Maintain participant files including case notes for pertinent data collection, ensuring confidentiality at all times.
To support the reconnection to home community and family if appropriate
Ensure program delivery is consistent with funding agreements, including community engagement and system navigation support
Evaluate the effectiveness of service and program delivery
Light cleaning duties
Organize, facilitate and/or attend community events to enhance participant and community knowledge of available services, supports and resources.
Promotes the values of the organization within the workplace and in the community by demonstrating positive and professional relationships with others (employees, citizens, council, volunteers, advocates, etc.)
Reports all unsafe conditions, hazards or practices, accidents and incidents in accordance with the Occupational Health and Safety Act and adheres to all Health and Safety requirements within the MNO’s policies and procedures and provincial legislation.
Performs other job/branch-related duties as required.
Qualifications:
1 to 3 years of experience
College Diploma (1 or 2 year) social services, social work, or human needs services
Client service experience
Experience in developing and facilitating presentations
Experience dealing with crisis situations in a professional manner
Knowledge of homelessness issues and barriers to accessing housing, especially within Métis and other Indigenous populations Advanced communications skills
Situational awareness
Organization skills and time/priority-management
Highly detail-oriented
Crisis intervention skills
Demonstrated ability to problem-solve, demonstrate good judgement, and make decisions independently within set processes and procedures
Demonstrated ability to work alone/independently with minimal supervision
