Job Description: POSITION PURPOSE SUMMARY: The Team Leader will plan and implement individualized programming while promoting an environment in which all supports and activities are built upon individualized and measurable goals. They are responsible to lead shifts – guiding and motivating the team to achieve organizational goals. ACCOUNTABILITIES: The incumbent may perform all, some, or similar duties, those listed below: Frontline Direct Client Support • Provide inclusive supports which can include treatment, or rehabilitation plans for individuals within the assigned Lawson Ministries programs Assist individuals in developing person-directed goals; helping track and monitor progress in conjunction with staff, family, and others involved in the individual’s life • Provide personal care to individuals as required • Assist with supporting individuals’ day-to-day living responsivities (i.e. household chores, meal prep, etc.) • Administer, accurately document, and order medications for supported individuals • Coordinate community-based activities and natural supports to foster community engagement • Navigate community resources to identify opportunities for service users to build skills and realize community-based goals Leadership of Team Culture • Assist with coordinating day-of staffing assignments based on available staffing resources and client demands • Check-in and communicate with team members as needed during the course of their shift; documenting any required action items • Completes daily on-call reports for the shift worked • Assist with any type of emergencies that may arise during the shift; and provide assistance to other programs as needed • Monitor level of program supplies; requesting re-order/purchase as needed • Ensure the AIMS Case Management software is used for all documentation and support team members in using the software Scheduling • Ensure program coverage in accordance with government, contractual and organizational requirements for assigned program • Complete scheduling in accordance with call down practices; resolving scheduling conflicts as they arise, and liaising with 3rd party staffing agency to coordinate substitutes as needed • Input requests from employees for time off including vacation requests in accordance with organizational policies • Distribute weekly schedules for assigned program • Coordinate with Supervisors and Administrative staff to schedule annual trainings for employees Quality Assurance Measures (QAM) • Complete physical site spot checks; ensuring proper documentation • Complete bi-weekly QAM checks for assigned location; follow up with Supervisors or Administration as required • Ensures front line staff maintain the general upkeep, cleanliness and maintenance of applicable programs spaces Perform other duties as required CRITICAL RELATIONSHIP MANAGEMENT Internal: • Directors, Supervisors, Co-workers, Front Line Staff, Volunteers & Clients External: • Community Members, Vendors, Client’s Family Members MANAGERIAL RESPONSIBILITY: • Reports directly to the Program Director or designate • This position has no direct reports FINANCIAL AND MATERIALS MANAGEMENT: • Uses or maintains organization assets in accordance with establish guidelines • Utilizes assigned equipment and supplies appropriately as per policies and procedures WORKING CONDITIONS: • This is a full-time position based on 40 hours per week • Schedule may vary according to requirements of responsibilities; weekend and evening work will be required • Hours of work and work schedule will be set in coordination with the employee’s supervisor • This position is not considered a management position for the purposes of the Employment Standards Act of Ontario • Regular travel between locations in the city may be a requirement of this position (within Hamilton, and surrounding area) • Position will be in-person due to the requirements of the position • Physical lifting of us to 20lbs may be required The above responsibilities must be discharged in accordance with The Salvation Army’s Mission Statement, in a professional manner, upholding our code of conduct. EDUCATION, EXPERIENCE, AND QUALIFICATIONS: Education, Qualifications and Certifications: • Completion of a formal post-secondary/college program of two academic years: Developmental Social Worker Diploma or equivalent in Human Services field • Current First Aid, Cardiopulmonary Resuscitation with Defibrillation Training, and current Non-Violent Crisis Intervention certification are required • Alternative combinations of education and experience may be considered. Experience and Knowledge: • Minimum of one (1) year of prior related experience working with adults with developmental disabilities and vulnerable populations is essential • Lifts and Transfer training and Personal Care Support experience are both considered an asset • Proficient in working in a computerized environment; must demonstrate strong skills using Microsoft Office Suite: Outlook, Word, Excel, Power Point. Skills and Capabilities: • Act as a role model for inclusive, respectful and ethical practice • Self-motivated/disciplined • Ability to maintain information in confidence and exercise good judgement • Demonstrated ability to work independently and participate as an active and responsible team member in a cooperative team environment • Excellent interpersonal skills, integrity, and adaptability • Ability to handle complaints and resolve conflicts • Exhibit good listening skills, have strong oral/written communication skills and respect authority • Participate in mandatory orientation training, on-going professional development and training • Participate in regular supervision and performance appraisal processes • Demonstrate an understanding of the psychological and social dynamics of working with individuals who experience complex, responsive behaviours. • Work in compliance with OH&S Act and Regulations and abide by The Salvation Army’s health and safety policies and procedures. • Represent the organization in a positive, professional, and engaging manner. • Comply with all Salvation Army policies, procedures, and associated legislation. • Treat the property of The Salvation Army with due care and caution. • Must have at minimum a valid Ontario Class “G2” Driver’s License, personal vehicle an asset, current copy of driver’s abstract that is satisfactory to The Salvation Army, in its sole discretion, is required • Ability to provide an original copy of a Background Check with vulnerable sector screening that is satisfactory to The Salvation Army, in its sole discretion. • Willingness to develop an understanding and support for the mission and purpose of The Salvation Army in Canada and its implications as related to position responsibilities. WAGE: $25.00 per hour Compensation: The target hiring range for this position is $23.28 to $29.10 with a maximum of $34.92. Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
