About SOAHAC
Southwest Ontario Aboriginal Health Access Centre (SOAHAC) is a multi-service Indigenous health and wellness organization providing primary care, mental health, traditional healing, and social supports for First Nations, Inuit, Métis, and urban Indigenous individuals across southwestern Ontario.
Our London site at 493 Dundas Street is an interdisciplinary care hub serving Indigenous community members in the city and surrounding area. We work within a wholistic, culturally safe framework that integrates Western clinical practice and Indigenous knowledge.
Role Overview
The Family Physician is a core clinical provider at the London site. Working collaboratively with Nurse Practitioners, Traditional Practitioners, Elders, Mental Health clinicians, Nursing, and Social Services staff, the Physician provides comprehensive primary care across the lifespan for SOAHAC clients.
The Physician supports longitudinal, relationship-based care and clinical decision-making while coordinating with internal and external partners to address client needs.
Key Responsibilities
•Provide comprehensive primary care for SOAHAC clients, including health promotion, disease prevention, acute care, and chronic disease management.
•Conduct assessments, develop care plans, provide follow-up, and coordinate ongoing primary care for SOAHAC clients
•Collaborate with Nurse Practitioners and other interdisciplinary providers within their respective scopes of practice.
•Participate in interdisciplinary case discussions, case conferences, and integrated care planning.
•Maintain accurate and timely clinical documentation in accordance with CPSO standards, organizational policies, and applicable privacy legislation (including PHIPA).
•Practice in a trauma-informed, culturally safe, and client-centered manner, recognizing the social determinants of health affecting Indigenous communities.
Qualifications
•Medical Degree (MD) and Certification in Family Medicine (CCFP), or equivalent.
•Licence for Independent Practice, or eligibility, with the College of Physicians and Surgeons of Ontario (CPSO).
•Experience working with Indigenous communities, community health centres, or underserved populations is an asset.
•Demonstrated ability to work effectively within an interdisciplinary, team-based model of care.
•Commitment to ongoing learning in Indigenous cultural safety, trauma-informed care, and anti-oppressive practice.
Compensation and Benefits
•Annual PSA salary of $331,975 for 35 hours per week, pro-rated for part-time arrangements.
•Participation in the Healthcare of Ontario Pension Plan (HOOPP) – defined benefit pension.
