About the role: Reporting to the Director of the Regional Cardiac Care Program at Southlake Regional Health Centre the Nurse Practitioner is an integral member of the Regional Cardiac Care Program (RCCP). Southlake’s RCCP is one of the four largest cardiac regional programs in Ontario and provides regional cardiac care within York Region, Simcoe, and South Simcoe. What you'll be doing: The Advance Practice Nurse/Acute Care Nurse Practitioner will: Provides high quality, safe, effective and evidence- based care in partnership with patients and families Uses advanced clinical knowledge and clinical practice expertise to promote optimal health and high quality of care. In this role, the HFNP works to full NP scope, is autonomous in accordance with the College of Nurses Of Ontario (CNO), Standards of Practice: Nurse Practitioners Will work collaboratively with the interdisciplinary team to optimize care delivery across multiple settings, including inpatients and outpatients Provide leadership and assist with professional development of interdisciplinary team members in the Cardiac Program through mentoring and participating in formal and informal education Development and evaluation of policies that promote quality care by implementing evidence-based practices Inform the development of key clinical pathways to improve the quality of care, reduce risks, increase patient satisfaction and increase the efficiency in the use of resources Will provide cross coverage within the cardiac program as required What you'll be doing: Master in Nursing/Nurse Practitioner Stream OR/ Master in Nursing/Master of Science in Nursing with successful completion of a post Master’s Acute Care Nurse Practitioner Certificate program from a recognized school of graduate studies Minimum 1 years’ experience as advanced practice nurse in CV Surgery preferred Minimum 3 years’ relevant clinical specialty-related experience as a Registered Nurse Current Member of the RNAO an asset Nurse Practitioner’s Association of Ontario Demonstrated ability to work in a collaborative practice model with inter-professional teams. Demonstrated ability to use initiative and work autonomously utilizing critical thinking skills, and advanced decision-making, problem recognition and problem solving skills. Experience in clinical nursing, inter-professional leadership and education. Experience with program development, quality improvement initiatives, and program evaluation. Effective organizational, interpersonal and communication skills, and demonstrated negotiation and conflict resolution skills.
