Position Summary Duties
- Assist patients with activities of daily living: This includes feeding, lifts and transfers, bathing, skin care, oral hygiene, and toileting.
- Perform tasks delegated by an RN or RPN in compliance with the RHPA for which appropriate education and training has been completed. RNs and RPNs will be responsible for the oversight of the PSW when performing the delegated tasks. Delegated tasks can include actions that involve insertion of a digit or instrument into a body cavity, or any care or procedure under the dermis, for example.
- Communicate and demonstrate basic information in accordance with a patient's pre-established Plan of Care.
- Ensure a safe environment for patients and staff by reporting and documenting unsafe conditions such as broken equipment as well as any patient behavioral, physical, or cognitive changes. Safety concerns must be reported to an RN, RPN or other regulated health provider, in compliance with patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patients.
- Complete and maintain related patient records and progress notes, as per hospital policy.
Qualifications
- Education: Successful completion of a PSW program from an accredited community college;
- Current Basic Cardiac Life Support (BCLS) Certificate;
- Current registration with the Health and Supportive Care Providers Oversight Authority (HSCPOA);
- Behavioral management training (such as PIECES, U-First, CPI) is considered an asset
- Ability to meet the physical demands of the job required including: ability to stand, stoop, walk, bend, lift, push, pull and reach without any physical discomfort on a continuous basis; transferring patients with appropriate assistance; ability to stand for extended periods of time; ability to lift in excess of 40 kg
- Acute care and/or previous rehab experience considered an asset;
- Good attendance record is required
- Recent six (6) months direct patient care experience
- Ability to speak, read, write and comprehend the English language
- Knowledge of the Occupational Health \& Safety Act and other relevant legislation
- Knowledge of the Workplace Hazardous Materials Information System (W.H.M.I.S.)
- Demonstrated independence, a positive attitude and the willingness to work effectively in a team
- Ability to facilitate unit functioning and smooth patient care flow
- Ability to maintain a consistent level of productivity and efficiency
- Ability to perform responsibilities in a courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards
- Awareness of Patient Centered Care
- Exercise initiative and good judgment with ability to multi-task
- Compliance with confidentiality requirements under UHN Privacy Policy
- Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand
