Duties
- Use information from individuals, families and referral sources to develop comprehensive intake psychosocial assessments for review by Ongwanada's Admission Review and Discharge Committee and for approval for services;
- Inputting intake assessment information into local or provincial data bases e.g. RASM, DCSIS, and data base for Children's Coordinated Service Planning;
- Complete Intake Coordination for DSO services for individuals within the organization and community;
- Collaborate with the inter professional team and community-based service providers;
- Support individuals and families in navigating the service Lead team communications with individuals and their families regarding services;
- Provide individual, group, family and couples therapy using the appropriate theoretical framework;
- Advocate and promote the interests and rights of individuals supported and their families;
- Provide clinical supervision to social work students and educating individuals and families about relevant topics;
- Performs other related duties as assigned/required.
For The Complete Position Description Please Contact Human Resources. QUALIFICATIONS, SKILLS \& KNOWLEDGE REQUIRED :
- Master of Social Work with five years direct clinical experience preferred; an equivalent combination of education and experience would be considered;
- Current registration with the Ontario College of Social Workers and Social Service Workers;
- Experience conducting psychosocial assessment and providing individual and family therapy;
- Understanding of case management principles, diagnostic criteria and clinical experience in assessment formulation and report writing;
- Experience working with individuals with developmental disabilities, including those with a dual diagnosis;
- Excellent knowledge of community resources;
- Valid G Driving License with full driving privileges, insurance and access to a reliable vehicle;
- Bilingualism (French and English) an asset.
