The Seniors Services Development Division requires staff to provide person-centered care to our clients and caregivers following the Butterfly Philosophy of Care. This will ensure high levels of emotional wellbeing and a better quality of life for those accessing our services. Staff are to have the emotional intelligence ability to deliver person-centered care in an authentic way that is meaningful to the clients and caregivers.

The Role
Working collaboratively with the interdisciplinary team, community partners, primary care providers and under the direction of the Supervisor, Adult Day and Community Support Services you will provide guidance and leadership to the Adult Day Service (ADS) team, clients, and caregivers. You will apply your professional knowledge and competent practice in the provision of quality, client focused care based on the College of Nurses of Ontario (CNO) and subsequent regulations, the Regional Values,
policies, procedures and best practice guidelines.
Responsibilities
- Work collaboratively with the Supervisor(s), Adult Day and Community Support Services and review admission files to determine suitability, coordinate the admission of clients, assess the clients' and family caregivers' needs and develop and provide individualized, client focused care plans
- Conduct thorough and regular nursing and health assessments of clients, provide internal referrals,monitor health conditions, update the plan of care and document care and services provided to the client and/or caregiver in the client's electronic health record
- Work collaboratively with the integrated care team and implement nursing skills within the integrated care setting
- Promote and support health education and knowledge transfer to enable clients and/or caregivers to make informed choices about health practices and healthy lifestyle choices that promote self-care
- Organize, coordinate, and conduct health education sessions on various topics such as foot care and medication management as needed
- Provide one to one support for clients and caregivers to assist with navigation of the health care system i.e. referrals to other community agencies and provide information on community resources
- Promote and maintain a safe and healthy working environment by adhering to proper procedures and prompt reporting of unsafe conditions and/or situations. Respond to and participate in emergency situations and emergency preparedness exercises and complete applicable staff/client incident reports as
Required
- Participate in the Continuous Quality Improvement program to evaluate and improve client services
Qualifications
- Current certificate of competence as a Registered Nurse with the College of Nurses of Ontario
- Current certification in CPR and first aid
- Minimum of 2 years' experience in geriatric and/or community nursing.
- Bachelor's of Science in Nursing (BScN) is preferred
- Certificate in Gerontology or equivalent, Rehabilitation, Medical/Surgical nursing would be considered an asset
- Experience in Health Education/Health Promotion and Program Planning including implementation, monitoring, documentation and evaluation
- Ability to facilitate group discussions with working knowledge of adult education principles
- Strong clinical skills and knowledge of senior care services and community resources
- Excellent written and verbal communication skills to effectively communicate with clients, caregivers, volunteers, staff and other professionals
- Strong leadership and interpersonal skills to work collaboratively with clients, caregivers, staff, and community agencies
- Strong customer and client focused approach in daily responsibilities
- Demonstrated ability to work effectively independently and/or within the interdisciplinary team
- Proficient computer skills
