Sinai Health is looking for an Advanced Practice Physiotherapist as a member of our Rehabilitation Department at Mount Sinai Hospital providing service to our Orthopaedic and Rheumatology patient population.
The Advanced Physiotherapy Practitioner provides leadership and program development for Sinai Health's ambulatory programs, including Hip and Knee Arthritis Program by fostering patient engagement for informed decision making, improving knowledge translation in patient education, incorporating technological and research advances in patient care, reducing length of hospital stay and actively mitigating risk in complex musculoskeletal conditions. This role promotes best practice guidelines for conservative management of medical symptoms, as well as post-operative care.
In this role you will:
Expert Clinical Practice:
- Apply advanced clinical skills to perform autonomous, comprehensive assessments that incorporate clinical background, medical history, physical examination, diagnostic imaging, and standardized outcome measures.
- Apply best practice to triage referrals, make recommendations and provide treatment options.
- Assess urgency and severity of the patient's condition and determining the appropriate care pathway with a range of options using an equitable and inclusive approach.
- Selectively order appropriate imaging and diagnostic tests under delegation.
- Follow up on test results, collaborate with physician when required and communicate with patient, where appropriate, to arrange further management.
- Provide education and recommendations to support best practice of condition management.
- Consult and collaborate with physician and care team to establish care plans for complex patient needs.
Program Evaluation and Research:
- Contribute to program evaluation and monitoring the model of care performance in terms of structures, processes, and outcomes to achieve a model of care that responds to the health system and patient needs.
- Achieving services that improve patient experience, reduces unnecessary referrals and investigations, and efficiently addresses patient's medical needs.
- Research and other forms of evaluation to address problems and issues arising from practice.
Leadership and Consultancy:
- Participate in processes to gather information and data required to support practice changes.
- Engage in organizational and regional networks to influence, share practice, and drive change for better patient care and health system efficiencies.
- Develop and deliver coordinated patient care.
- Contribute to the development, evaluation and improvement of team-based care and practice innovations.
Professional Development and Education:
- Identify individual learning needs and plans to address gaps, adopting a structured approach to continuing professional development.
- Participate in the design and delivery of learning opportunities for various health care providers both within and outside the organization, including ACPAC trainees, medical and rehabilitation (physiotherapy and occupational therapy), and/or nursing students, international observers, and other trainees.
- Use mentoring and peer support to develop competency for the advanced practice role; Collaborate with the broader team to identify opportunities for shared learning and pursue academic linkages.
Job Requirements:
Mandatory:
- Successful completion of Master's degree or completion of a University degree in Physiotherapy/Occupational Therapy/Nursing/Chiropractic or equivalent.
- Completion of the Advanced Clinician Practitioner in Arthritis Care Program (ACPAC).
- Minimum of five (5) years of recent and related experience.
- Licensure, in good standing, with associated Health Discipline College in Ontario.
Skills and Knowledge:
- Expert-level arthritis, musculoskeletal, and systemic autoimmune rheumatic disease assessment skills to diagnose a disease, physical disorder, or dysfunction as the cause of a person's symptoms and signs.
- Proven strong leadership and collaboration skills.
- Knowledge of best practices in rheumatic and musculoskeletal care/treatment and evidence-informed decision making.
- Experience and skill in complex decision-making processes.
- Collaborative approach to teaching/learning in an interprofessional environment.
- Excellent interpersonal, verbal and written communication skills.
