Detailed OverviewReporting operationally to the Manager, Clinical Operations, and receiving clinical supervision from the Clinical Practice Leader, the Social Work Assistant works in a team based environment and provides social work support services during the admission and discharge of patients/clients. Duties include interviewing clients/families/facilities for information, assisting clients to approach external resources, providing practical support to clients regarding benefits, housing, transportation, community resources, finances, and acquiring and maintaining a supply of up to date information and applications on services/agencies/resources.
Responsibilities - Works closely with designated Social Worker(s) to provide social work support during the admission and discharge of patients/clients, by methods such as providing relevant resource information, completing Release of Information and application form with patients/clients, and supporting patients to arrange transportation home when requested. Shares appropriate information with the interprofessional team regarding clients and their caregivers/families.
- Interviews patients/clients to obtain information by methods such as screening patients/clients to establish eligibility for community resources and benefits, contacting family and agencies to obtain additional patients/clients information and refers patients/clients/families to other appropriate resources as required.
- Assists patients/clients with applying to external resources concerning housing options, income, transportation, and community supports by providing contact,information, brochures and applications and ensuring client applications are completed and processed.
- Provides practical support to patients/clients by methods such as assisting clients with completing forms, writing letters, obtaining necessary consent for release of information, arranging transportation, applying for and mailing applications for benefits and providing clients with information regarding programs that they have applied to.
- Acquires and maintains a supply of application forms for various services/agencies/resources as well as procuring and maintaining housing applications. Collects, organizes and maintains up to date information on federal, provincial and community resources/services and assists clients with access to this information.
- Assists in the development of resource materials for patients/clients/families such as Home Health Services, Community Services, Financial Resources and Mental Health and Substance Abuse resources.
- Participates in activities that facilitate patients/clients goal setting in an inter-professional and collaborative manner, and is able to effectively problem solve, and resolve conflict.
- Completes required reports and maintains update patients/clients records on assistance provided and resources applied in accordance with established procedures for documentation and other organizational reporting purposes.
- Performs other related duties as assigned.
QualificationsEducation, Training and Experience
Completion of a recognized Social Service Worker or Social Service Certificate Program or an equivalent combination of education, training and experience.

- Physical ability to carry out the duties of the position.
