Position Summary
The Nurse Practitioner in the Seniors Health Centre primarily provides clinical care to support Long-Term Care (LTC) patients at SHC. The NP collaborates with physicians, navigators, coordinators, and other healthcare team members to prevent avoidable ED visits and hospitalizations, support hospital transitions, and act as a link between hospital and SHC. The role involves supporting program development, ensuring high-quality, safe care, and educating team members on best practices for geriatric patient management. The NP also contributes to NYGH's Ontario Health Team mandate to improve population health through coordinated, integrated care.

Practical Responsibilities
- Practice according to relevant standards and legislation
- Support hospital admission processes for LTC residents with complex needs
- Assist in reducing non-urgent ED transfers
- Provide assessment, intervention, and follow-up for acute and chronic illnesses
- Lead and promote excellence in patient-centered care
- Participate in regional communities of practice and clinical pathway development
- Coordinate with external partners for continuity of care
- Address medication reconciliation issues upon hospital discharge
- Contribute to Quality Improvement initiatives
Qualifications
- Master's degree in Nursing/Nurse Practitioner from a recognized institution
- Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP
- Basic Cardiac Life Support (BCLS) certification
- 1-3 years of NP experience preferred; LTC experience is an asset
- Knowledge of LTC legislation and gerontology is preferred
- Strong communication, leadership, and interpersonal skills
- Ability to work independently and as part of a team
- Excellent time management and assessment skills
