Job Description
Position Purpose: The Relief Case Worker will develop and implement goal-oriented programs, providing support within workplace and residential settings for adults with developmental disabilities participating in the CPS Lawson Ministries program and community activities. The role requires flexibility to work evenings and weekends, with shifts scheduled by the supervisor.
Main Responsibilities:
- Promote independence and enhance quality of life for Service Users.
- Create supportive, nurturing environments that foster independence.
- Assist Service Users with emotional and psychological needs, involving them in planning and evaluation of personalized plans.
- Ensure physical needs are met and provide personal care as needed.
- Support community inclusion and participation.
Additional Responsibilities:
- Maintain knowledge of person-directed plans, policies, and procedures.
- Organize transportation for Service Users.
- Support spiritual needs through participation in programming.
- Work collaboratively with team members and support networks.
- Develop therapeutic relationships with Service Users and families.
- Accurately document all activities using AIMS software.
- Administer and document medications following training.
- Maintain cleanliness and safety of the workspace.
- Respond to emergencies as needed.
Qualifications
- Post-secondary diploma in Developmental Social Work or related Human Services field.
- Experience with community resources and vulnerable populations.
- Current First Aid \& CPR, CPI certification, and vulnerable sector criminal check.
- Valid Ontario Class G driver's license (vehicle accessibility preferred).
Experience \& Skills
- At least 2 years working with adults with developmental disabilities.
- Medication administration experience.
- Ability to lift up to 20 lbs and perform lifts/transfers.
- Strong communication skills and self-motivation.
- Ability to work independently and in teams.
Compensation
Target range: $21.83 to $27.29 per hour, with a maximum of $32.75, based on experience and qualifications.
