We are seeking a Registered Social Worker to join our SCOPE Mental Health Service. This role serves as the primary contact between Primary Care Providers (PCPs), patients, and psychiatrists, providing assessments, resource linkage, brief case management, and informal bridging counseling. The position involves patient follow-up, advocacy, and collaboration with healthcare teams.
Employment Status: Temporary Part-Time
Duration: March 31, 2026
Hours of Work: Day shifts -- Monday to Friday (3 full days Week 1, 4 full days Week 2)
Location: Wilson Site -- Cross Site Coverage may be required

Reporting Relationship: Manager of Mental Health
Hourly Rate: $44.67 - $57.90
Responsibilities
- Assess patients to evaluate needs, problems, and formulate treatment plans
- Assess acute/crisis needs and triage for diagnosis and treatment recommendations
- Assist PCPs and patients with service navigation and resource connection
- Provide brief, time-limited individual support via telephone/video/in-person
- Utilize Brief Solution Focused counseling principles
- Apply recovery and self-help principles
- Liaise with community agencies
- Collaborate with HRH consulting psychiatrists
- Summarize interventions and recommendations for PCPs
Qualifications
- MSW from an accredited School of Social Work
- Current membership with OASW and OCSWSSW
- Certification in managing aggressive behavior (asset)
- 2-3 years recent experience in mental health/addiction settings
- Knowledge of community mental health resources
- Ability to work independently and as part of a team
- Skills in individual counseling
- Excellent problem-solving, interpersonal, and communication skills
- Organizational and time management skills, adaptable to a fast-paced environment
- Experience with Trauma-Informed Care
- Cross-cultural awareness and sensitivity
- Proficiency with electronic medical records and MS Office
- Comfort with technology-assisted healthcare delivery, including video conferencing
- Good attendance record
- Knowledge of policies, legislation, and social welfare services
