Job Summary: Utilizing clinical knowledge and skills, Social Workers develop collaborative relationships with aging patients and their families to promote patient-centered goals. The role involves psychosocial assessments, discharge planning, referrals, resource navigation, advanced care planning, and counseling, focusing on ensuring successful transitions in care. The Social Worker will facilitate referrals to Ontario Health at Homeservices and community resources, acting as a clinical resource within the interprofessional team. This position reports to the Manager and Interprofessional Practice Chief for Social Work.
Responsibilities include:
- Provides psychosocial assessments, crisis intervention, discharge planning, and counseling to patients and families.
- Supports and consults with other disciplines on psychosocial issues, especially in complex situations.
- Leads discharge planning and patient flow initiatives.
- Serves as liaison with Ontario Health at Home.
- Acts as a communication conduit between families and the care team.
- Establishes community linkages and outreach programs.
- Participates in teaching and supervision of health professional students.
Qualifications include:
- Masters of Social Work degree.
- Current registration with the Ontario College of Social Workers and Social Service Workers ('OCSWSSW').
- Experience in discharge planning and system navigation preferred.
- Healthcare setting experience preferred.
- Interpersonal, communication, and problem-solving skills.
- Basic computer skills -- Meditech, Microsoft Word and Excel; RM\&R preferred.
