Job Description:
This position is with Extendicare, located at Bayview LTC in Toronto, Ontario. The role involves acting as a Behavioral Supports Ontario (BSO) program expert, ensuring program goals are met, and managing staff performance related to behavioral support initiatives.
Key Responsibilities:
- Coordinate interdisciplinary team meetings and collaborate with social work, pharmacy, and physicians.
- Participate in advisory and quality committees.
- Complete reports, statistics, and documentation related to behavioral support.
- Set goals, create action plans, and follow up on progress.
- Analyze data and present reports on quality indicators and medication use.
Qualifications:
- Currently registered as an RPN in good standing with the College of Nurses of Ontario.
- Current CPR and First Aid Certificates.
- Knowledge of P.I.E.C.E.S.TM, Gentle Persuasive Approach, U-First!, Montessori.
- 3-5 years of experience in long-term care.
- Experience working with individuals with dementia, delirium, mental health issues, addictions, neurological conditions, or behavioral changes.
- Commitment to the principles of the BSO program.
- Canadian certificate in Gerontology is an asset.
