Job DescriptionThis is a unique opportunity to be part of a tight-knit, collaborative team from across the health sector that will deliver the next generation of palliative care in Mississauga, providing holistic proactive, seamless care and support to patients, families and caregivers – enhancing quality of life, comfort, dignity and security in patients’ end of life journey. As an integral member of the interdisciplinary care team, the Nurse, Palliative Care provides holistic, patient-centered care that includes symptom management, coordination of care, patient, family and caregiver education, emotional support, and advocacy for assigned patient(s) and/or caseload of patients.Works under the supervision of the Neighbourhood Home Care Specialist.Collaborates with the interdisciplinary team to create personalized care plans based on the patient’s unique needs and goals of care.Participates in clinical huddles and case conferences with team members such as physicians and allied health.Practices in accordance with the College of Nurses of Ontario (CNO), the Regulated Health Professions Act and Spectrum Health Care standards, policies and procedures.Travels within assigned region between patients’ residences or facilitiesQualificationsCurrently registered and in good standing with the College of Nurses of Ontario.Minimum of two-years nursing experienceMust have experience working with palliative care patientsSuccessful completion of any of the following is considered an asset:Canadian Nursing Association (CNA) certification in Palliative Care Nursing or enrollment in CNA certification program in Palliative Care Nursing; orLearning Essentials Approaches to Palliative and End of Life Care (LEAP); orFundamentals of Palliative CareThe Comprehensive Advanced Palliative Care Education (CAPCE)Current police check suitable for working with vulnerable sectorValid Ontario driver's license and reliable vehicle with willingness to travelDemonstrates problem-solving and critical thinking skillsProficiency in English both written and verbalAbility to work independently within the scope of the role and prioritize workload.
