POSITION SUMMARY
The part-time Youth in Transition Worker (YIT) supports youth aged 15-23 who are transitioning from CAS care/supports to independent living. The program is funded by the Ministry of Children, Community, and Social Services. It provides direct support, advocacy, and resource navigation to assist youth in securing stable housing, education, employment, and life skills development. Program goals: reduce the intensity of involvement in the child welfare system, provide more equitable access and enhance linkages to community services. The YIT Worker flexes their schedule to accommodate youth needs and support as appropriate.
KEY RESPONSIBILITIES
Direct Support:
Engage youth and build rapport using flexible, youth-centred approaches.
Support youth to identify their goals or needs and work on the steps to implement them.
Assess strengths and barriers with youth.
Advocate for youth access to community resources and opportunities when appropriate.
Provide transportation and accompany youth to appointments as needed.
Facilitate crisis intervention, mediation, and conflict resolution between the youth and their ancillary contacts as needed.
Model healthy relationships and problem-solving strategies.
Maintain a non-judgmental and inclusive approach to youth engagement.
Utilize de-escalation skills and strategies to support youth.
Teach and support related life-skills development.
Case Management and Documentation:
Conduct assessments to determine youth needs and develop support plans.
Maintain detailed case notes and progress reports.
Collaborate with CAS workers and other service providers.
Ensure timely and accurate reporting of program outcomes.
Manage a caseload of approximately 10 clients annually.
Resource Development and Community Outreach:
Develop and maintain resource guides and referral contacts for youth services.
Promote the YIT program within local community organizations.
Establish and foster positive working relationships with internal programs, external agencies and services, and CAS workers.
Represent JHS in community meetings and youth-focused initiatives.
Administrative Responsibilities:
Follow agency policies, procedures and collective agreement.
Ensure timely communication and reporting of client progress.
Maintain and submit statistical data and administrative forms as required.
Attend and contribute to program, agency and community meetings and training as appropriate.
Ensure calendar is updated.
QUALIFICATIONS AND EXPERIENCE Education & Certifications:
Post-secondary diploma or degree in Social Services, Child and Youth Work, Social Work, or a related field.
Class G license and access to a reliable vehicle with $2,000,000 liability insurance.
Valid First Aid/CPR certification (asset).
Crisis intervention and ASIST (asset)
Experience:
Experience working with at-risk youth.
Knowledge, Skills, and Abilities:
Ability to maintain flexible work hours.
Strong ability to engage youth and build rapport quickly.
Strong ability to work from trauma-informed, anti-oppressive and culturally inclusive frameworks.
Ability to adapt processes, communication, schedule, etc. to accommodate youth.
Strong ability to effectively utilize de-escalation, active listening and communication skills.
Strong ability to maintain professional boundaries and confidentiality.
Some knowledge of child welfare system and awareness of community resources.
Ability to work within a team and work independently.
Understanding of equity, diversity, and inclusion practices.
Proficiency in Microsoft Office and data management tools.
WORKING CONDITIONS
Office and community-based work environment.
Part time hours (15 hours per week) with occasional evening flexibility.
Interaction with youth, CAS workers, other agency personnel, and community partners.
Manual dexterity required for computer and office equipment use.
