What is the Guelph CHC?
The Guelph CHC is located in the vibrant City of Guelph, Ontario, and provides primary care services and community health programs using an interdisciplinary, team-based approach. We have innovative client-centered services delivered by interdisciplinary teams. Our clinical team consists of family physicians, nurse practitioners, and registered practical nurses, who work collaboratively with allied health workers, including social workers, dietitians, outreach workers, and cultural interpreters.
The Guelph Community Health Centre works within a determinants of health framework which emphasizes health promotion and education, disease prevention, anti-oppression, and recognition of special risk factors (social, physical, psychological, etc.) which may affect the health of clients. This position is located primarily at the downtown site of the Guelph CHC.

Primary Responsibilities:
Provide comprehensive, harm reduction focused, non-stigmatizing, wraparound health care and substance use options for people with complex physical, mental health and substance use needs.
Work with an interdisciplinary team (including Nurse Practitioners) to offer assessment and treatment, preventative care, episodic health care, primary care and substance use health care.
Use a low barrier, compassionate approach to providing health care, meeting clients where they are at, considering equity and lack of access to social determinants of health, including income, housing, and food.
Offer both appointment-based and walk-in care, working with the allied health team to reduce barriers to accessing health care
Attend daily huddles to share and receive updates about the program, the organization and/or client care
Participate in Guelph CHC’s on call rotation as required (consisting of after-hours phone availability) and occasional home visits as necessary
Attend meetings as requested to support program development, evaluation and improvements
Ensure high quality and safe care is delivered by maintaining complete and accurate medical records, participating in chart reviews and support the implementation of new guidelines and Medical Directives
Qualifications:
Licensed Family Physician, as per the requirements of the Ontario College of Physicians and Surgeons, with CCFP certification
Possess or is eligible to obtain required malpractice coverage with CMPA (this coverage is eligible for reimbursement)
Experience working with communities and individuals who face barriers to accessing health care services (e.g. clients experiencing homelessness, mental health concerns, using street drugs etc.)
Previous experience in community health/social services and/or a values-based organization is an asset
A demonstrated commitment to health equity and alignment with the CHC model of care
Proficient in the use of software applications including Outlook and electronic medical record systems
