You have clinical skills and knowledge of wound management, infusion therapy, palliative care, and chronic disease management.Provide patient-focused, safe, holistic nursing care and work collaboratively with other members of the health care teamEnsure that nursing care provided to our patients is based on best practices to achieve successful outcomesContinually assess, anticipate, identify and respond to new and changing needs of our patientsTravel within your region between patients’ homesQualificationsMinimum two years nursing experience (Community Nursing experience is ideal) Registered and in good standing with the College of Nurses of Ontario (CNO)Ability to work independently in the communityProficiency in English, both verbal and written required (additional languages an asset)Valid Ontario driver's license and reliable vehicleExperience with infusion therapy, mental health, palliative care, and wound careExcellent patient assessment and teaching skills#TO_nurse
