Reporting to the Site Leader/Director of Care, the Resident Care Manager is responsible for collaborating with the inter-disciplinary care team members in the provision of care and services for Residents of the home based on the Park Place Seniors Living (PPSL) Care Model and residents’ choice. Coordinates and supervises the care team and provides guidance, direction and discipline as required to ensure the delivery of resident-centered care based on residents’ individualized care plans and PPSL policies and procedures. The Care Manager demonstrates knowledge of leadership and of mental health/gerontology principles and best practices in accordance with the applicable provincial registering body, any provincial or local health services mandate and PPSL policies and procedures. The Care Manager will ensure a commitment to resident safety and to our “culture of safety” as per PPSL Strategic Directions.
This is a permanent full-time position working 37.5 hours per week.

QUALIFICATIONS:
Completion of the following programs: Bachelor’s degree in Registered Nursing and/or Psychiatric Nursing
College of Registered Nurses of Alberta C.R.N.A or College of Registered Psychiatric Nurses of Alberta C.R.P.N.A registration in good standing
2 or more years of experience in a managerial role is an asset
3 years of recent related experience or an equivalent combination of education, training, and experience.
Clear criminal record security clearance with Vulnerable Sector included (no more than 6 months old and must be from RCMP or a Municiple Police Service)
TYPICAL DUTIES :
Recruits, leads and supervises the interdisciplinary team to ensure effective, consistent, and comprehensive resident care planning and delivery based on residents’ choice and PPSL policies and procedures.
Ensures holistic, comprehensive assessments of the residents are completed on admission and ongoing basis for individualized care planning and delivery.
Provides leadership through a respectful, collaborative approach to monitor, supervise and direct the interdisciplinary team in care provision. Assists with new staff orientation and routine performance appraisals as allocated. Offers staff mentoring, education and/or discipline as required up to and including termination.
Works closely with professional nurses and HCA staff.
Acts as a role model by demonstrating professionalism, empathy, trust and respect in all interpersonal relationships, and recognizes personal impact on team dynamics.
Collaborates with members of the PPSL interdisciplinary team or outside agency as needed to ensure effective, consistent, and comprehensive care planning actions and delivery based on Resident centered care and choice and PPSL policies and procedures.
Supports staff in advocating for resident choice and autonomy in decision-making and care planning.
Monitors interdisciplinary team collaboration and processes to ensure quality resident-centered care and addresses issues as required.
The Care Manager will be required to carry the on-call phone on a rotational basis to assist the staff with emergent/urgent situations.
Ensures all documentation of assessments, records of observation, care provision and resident responses are according to professional standards and PPSL policies and procedures.
Leads the care team in:
(a) Planning, organizing and establishing priorities,
(b) Using resources effectively and efficiently,
(c) Responding to unanticipated events and changing resident needs,
(d) Reassigning clinical/health related tasks as necessary,
Leads the interdisciplinary care team in focusing on safety through exhibited:
(a) Accident prevention and reporting unsafe work conditions,
(b) Knowledge of emergency procedures and regulations,
(c) Using skills, supplies and equipment safely and appropriately,
(d) Utilizing infection control procedures,
(e) Committing to resident safety and to our “culture of safety” as per the PPSL Strategic Directions and Home goals.
Presents and/or attends in-service and other educational programs as required to maintain current clinical competence and knowledge.
Performs other related duties as assigned.
