The OT is responsible for conducting assessments, developing and delivering treatment plans focused on achieving the goals of care including promoting independence, enhancing comfort, and delivering education to support daily functioning and safety at home. Works under the supervision of the Program DirectorCollaborates with the interdisciplinary team to create personalized care plans based on the patient’s unique needs and goals of careParticipates in clinical huddles and case conferences with team members such as physicians, nurses, social workers, speech therapists and physiotherapistsConducts thorough assessments of cognitive, functional, and psychosocial abilities to facilitate effective communication and engagement for individuals Identifies and assists individuals in acquiring appropriate assistive technology. Modifies existing technologies to meet individual needs, ensuring optimal usageEffectively educates caregivers in necessary skills for assisting with personal care and safe transfer techniques, enhancing the care environment for patients and their familiesMaintains accurate and detailed documentation, including assessments, treatment plans, progress notes and discharge summariesQualificationsMaster’s degree in Occupational TherapyRegistered in good standing with the College of Occupational Therapists of Ontario3-5 years of experience working as an Occupational Therapist preferredValid Ontario driver's license and a reliable vehicleAbility to work autonomously and as part of an integrated health care team, with an emphasis on collaborative care planningProficiency in conducting assessments, interpreting findings, and formulating evidence-based treatment plans.Familiarity with assistive devices, adaptive equipment, and environmental modificationsAbility to write detailed occupational therapy consultation reportsProficiency in computer software programs (e.g., Microsoft Office) and virtual communication platforms (e.g., Microsoft Teams, Zoom, Go To Meeting)#palpeel_2Additional InformationSpectrum Health Care is thrilled to be named one of Canada’s Best Managed Companies.
