Position Summary
• Responsible for counselling patients and families dealing with paediatric diabetes as a member of a multi-disciplinary Paediatric Diabetes team. You will participate in clinic visits with patients and also provide 1:1 sessions.
• Responsible and accountable for decisions and actions in the provision and management of patients with diabetes and associated co-morbidities within the scope of practice of Social Work as defined by OCSWSSW as a collaborative member of the interdisciplinary team
• Practitioner, consultant, advocate and educator to meet the complex needs of a patients with diabetes, their families and the inter-professional team members
• Perform psychosocial assessments, case history information, short-term therapeutic intervention/counseling, to individuals living with diabetes and develop a goal oriented social work treatment plan and interventions; provide patient/family education and support to address psychosocial, emotional and financial needs of patients and families
• Provide referrals based on assessment
• Participate in program planning and resource development
• Participate in fostering community linkages
• Participate in in-service programs, program planning, Best Practices projects, Social Work Professional Practice Group, and hospital committees as required
• Develops education materials to meet the needs of the clients
• Participates in program planning and evaluation
Key Qualifications:
• M.S.W. with experience in supporting Paediatric patients and/or patients with diabetes
• Membership in OASW and registration in OCSWSSW
• Clinical experience in a hospital setting, with outpatient hospital experience preferred.
• Ability to perform in a navigational role with the ability to link patients with ongoing/long term social work and mental health support services.
• Experience and understanding of relevant community resources and how to access these
• Understanding of physical demands and emotional challenges faced by patients and their families living with diabetes
• Understanding of supports required by people living with diabetes and other comorbidities
• Excellent interpersonal, communication and organizational skills
• Reliable and flexible individual to work as a member of the interdisciplinary team
• Proficiency in computer, experience with electronic medical record and good keyboarding skills are essential
