The Middlesex-London Health Unit has exciting opportunity for one temporary full-time Outreach Worker until December 2025, to work on the Sexual Health team at our Citi Plaza, London location. Flexible work hours are required (including working Monday to Thursday evenings). KEY RESPONSIBILITIES: Under the direction of the Manager, Sexual Health the incumbent will: Monitor client’s health condition and environment by observing functional ability, living arrangements, and family supports, documenting behavioural, physical, and cognitive changes in accordance with standard procedures/protocols or specific directions. Provide advocacy, debriefing and supportive listening at time of diagnosis. Assist clients in accessing assistance for the prevention and transmission of communicable diseases. Act for the client in an advocacy role with other community agencies and health care providers. Attend meetings with community partners and participate in patient care planning and co-ordination. Carryout responsibilities in accordance with Middlesex London Health Unit occupational health and safety requirements. Provide clinical care, counseling, and street-level outreach services within the community. Arrange, plan, escort and accompany as necessary, designated/marginalized individuals on medical appointments and errands, while also providing emotional, moral, and physical assistance. Create linkages to other community resources and programs, contact relevant organizations/businesses/services, assist clients with processing forms. Act as a liaison between various partner/community agencies. Assess potential emergency situations and make appropriate interventions. Identify clients’ needs for services and make appropriate referrals, or develop alternate strategies for client support, education, and follow-up care. Perform administration such as responding to calls/enquiries, maintaining clients’ files, input data into computer software applications; and, Perform other duties as assigned. QUALIFICATIONS: Diploma in Community Social Services (2-year program) or equivalent Two to three (2-3) years related experience working with people who use drugs, individuals with HIV/AIDS, HEP C, and other Diseases of Public Health Significance, mental health issues and/or those involved in the sex trade, members of the Lesbian, Gay, Bi-sexual, Transgendered, Transsexual, Queer, Questioning Two Spirit, Asexual and Intersex (LGBTTQQ2SAI) community, Indigenous people and communities and/or an equivalent combination of education, training and experience. Minimum 1-year related experience and demonstrated ability to work with marginalized and culturally diverse populations including those with chemical dependency, mental illness and who are HIV positive. Experience working in a harm reduction or needle syringe program preferred. Demonstrated listening and information seeking skills that promote communication and lead to a cooperative approach to problem solving within a multidisciplinary setting. Demonstrated ability to establish rapport with clients to explore issues that are beyond the initial presented concerns. Experienced in motivational Interviewing skills an asset. Working knowledge and familiarity with health and social service programs in London Ontario, particularly the downtown area. Working knowledge of harm reduction principles. Working knowledge of assertive engagement model of care and related tools. Crisis prevention/intervention and conflict resolution/de-escalation skills. Excellent interpersonal and organizational skills. Proven ability to work independently and collaboratively in a team environment as well as capacity to work in partnership with other community agencies. Valid Ontario driver’s license and access to reliable transportation. Standard First Aid and Cardiopulmonary Resuscitation (CPR) Proficient technical skills in the areas of documentation in Microsoft Office and using Electronic Medical Records. Demonstrated commitment to contributing to a positive workplace environment. Communication proficiency (written and verbal) in French and/or any other language(s) is highly desirable. The hourly rate for this position ranges from $34.34 to $44.64 based on qualifications and experience.
