Detailed OverviewProvides personal care and/or assists patients with activities of daily living, including assisting with meals and feeding of designated patients; provides one on one personal care and observation of designated patients as directed by the Registered Nurse/Registered Psychiatric Nurse (RN/RPN); reports observations of patients' conditions to designated nursing staff; facilitates patients' participation in psychosocial rehabilitation activities; accompanies patients to offsite appointments and transports patients by operating passenger vehicle as required.
Responsibilities
- Provides personal care and/or assists patients with activities of daily living such as bathing, dressing, personal hygiene, prosthetics and elimination.
- Assists patients in making beds, organizing personal belongings, personal laundry as assists patients in retrieving personal care supplies from storage area as required.
- Assists patients with meals by serving and collecting meal trays; monitors patient safety and assists with feeding of designated patients.
- Weighs patients, monitors patients' nutritional/fluid intake and elimination and collects specimens as directed. Charts information and reports problems and/or changes to designated nursing staff.
- Provides one on one personal care and observation of designated patients as directed by the Registered Nurse/Registered Psychiatric Nurse (RN/RPN).
- Assists nursing staff with care of patients in seclusion rooms as directed by the RN/RPN including assisting with activities of daily living and delivering food.
- Reports observations of patients' conditions to designated nursing staff in accordance with established procedures/practices and records observations on the patient health care record.
- Participates in team meetings and ward rounds as directed and provides feedback on observations related to patient care provided for designated patients.
- As directed by the RN/RPN or Occupational Therapist, facilitates patients' participation in psychosocial rehabilitation activities by methods such as setting schedules and participating in scheduled individual and group activities.
- Escorts patients off unit for treatment or diagnostic procedures; transports patients utilizing mechanical aids such as wheelchairs and/or stretchers.
- Accompanies patients to offsite appointments to provide assistance as required.
- Participates in orientating patients, families, staff, volunteers, and students to the unit by familiarizing them with the layout of the unit, patient rooms and unit routines
- Assists care team without routine search of rooms and client belongings in order to maintain unit safety.
- Assists patients with retrieval/storage and/or relocation of personal effects to different rooms within the unit.
- Participates in quality improvement activities as assigned by providing input and feedback on department initiatives.
- Performs other related duties as assigned.
QualificationsEducation and ExperienceGrade 10 plus graduation from a recognized Mental Health Care Worker Program or an equivalent combination of education, training and experience.
