Overview
The Case Manager II delivers intensive, client-centered case management to Guests with complex needs, including those experiencing homelessness, substance use, and mental health challenges. Using a trauma-informed, harm reduction approach, the Case Manager II coordinates care and supports through collaboration with internal teams and external partners to promote housing stability, health, and well-being.
What You’ll Do
Engage Guests with high-acuity needs using a respectful, trauma-informed approach.
Provide tailored, housing-focused case management that supports autonomy and well-being.
Coordinate services across teams and systems for continuity of care.
Build trust-based relationships while maintaining ethical boundaries.
Support Guests in achieving goals related to housing, health, and life skills.
Maintain accurate, timely documentation in the electronic case management system.
Participate in team meetings and collaborative care planning.
Organize case consultations and coordinate with external partners.
Support crisis management through safety planning and de-escalation.
Supervise BSW student placements as needed.
Uphold policies on Guest/workplace safety and contribute to quality improvement.
Perform additional duties as required.
What You Bring
Degree in Social Work required; registration with NLASW
At least 2 years' experience in case management or a related role with marginalized populations.
Knowledge of local/provincial services and housing resources.
Strong interpersonal skills and understanding of systemic barriers.
Skilled in service navigation, planning, and conflict resolution.
Proficient in documentation and professional communication.
Adaptable, creative problem solver in dynamic situations.
Clear Record of Conduct and Vulnerable Sector Check required.
Equivalencies in education and experience will be considered.
