We are opening the doors to a state-of-the-art treatment hub designed to provide comprehensive support and resources that address the root causes of homelessness and addiction. We are currently seeking a 1 FTE System Navigator who will be responsible for working with an interdisciplinary team meet client’s health and social needs. The System Navigator will support clients with navigating health/social systems, referrals for services, obtaining identification, housing needs, and treatment applications.
One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services. The position has a competitive salary, the Health Care of Ontario Pension Plan and an excellent group health benefit package.
QUALIFICATIONS:
Bachelor’s degree in Social Work (HBSW), or equivalent relevant experience.
Ability to effectively engage clients in assessment of needs and goal planning.
Knowledge of and connections with community services and partners, including but not limited to health and social services, the judicial system and Indigenous organizations.
Demonstrated ability to facilitate and advocate for services to support clients.
Knowledge and background in the areas of trauma-informed care, culturally safer care, and working with individuals who use substances.
Sensitivity to the needs of a multicultural community and the impacts of social, economic and cultural issues on health and well-being.
A current driver's license and access to a vehicle to travel to designated outreach sites.
