Partners Community Health (PCH) is a new not-for-profit organization, focused on bringing healthcare services together around the needs of people living in Mississauga and West Toronto. PCH currently operates two new state-of the-art LTC homes and community hub in West Mississauga. The new homes have a combined 632-beds and be part of PCH’s larger strategy that introduce innovative and inclusive programs and services and new models of care delivery. Position Overview:
The Associate Director of Care will ensure residents receive excellent care and services, while ensuring operational excellence for the resident home area. The Associate Director of Care will lead the nursing team with all care activities, ensures accurate completion of all documentation and is a key liaison for all residents and family members.
Key Responsibilities and Activities:
Resident Care & Services:
To plan, direct, develop, co-ordinate, implement and evaluate resident care services; Oversees clinical care processes to ensure that care is delivered according to individual plans of care for residents.
Liaise with hospital, doctors, external providers, and available interdisciplinary services to ensure the most effective provision of care delivery in the home.
Leads the implementation and monitoring of required clinical programs, i.e., Falls Prevention, Skin and Wound Care, Continence Care, Pain Management
Actively participants in committees according to the terms of reference and areas of responsibility
Upholds accreditation standards and ensures that program and practice standards are consistent with professional guidelines; ensures all team members are following all best practise, legislation, and professional guidelines.
Working to oversee the development, implementation, review, and evaluation of resident care plans; ensures all RAI-MDS assessments are completed on time, all care conferences are held, and information is captured for discussion and ensures all data is ready for submission in partnership with the RAI coordinator and CIHI
Operational Excellence:
To ensure quality care and a safe, effective, and efficient work environment.
To assume responsibilities of the Director of Care as required in their absence.
To be knowledgeable of the Resident Bill of Rights, LTCHA and associated regulations and always promotes a resident first philosophy.
Ensures resident and family satisfaction and continuous quality improvement initiatives are promoted.
Coordinates and ensures care processes for the RAI MDS Program, Nursing Rehab, and Restorative Care Program are implemented, including evaluation outcomes, and monitoring of performance metrics.
Leads in the coordination and implementation of best practice and risk management initiatives related to resident care.
Conducts regular audits and in-services to ensure continuous quality improvement in conjunction with other leaders and departments.
Continuous collaboration with the interdisciplinary team to ensure a resident centred care approach to all service and care offerings; regular communication with residents and loved ones
Human Resources:
With support of the Director of Care, hires, directs, supervises, evaluates, and performance manages personal support workers and registered team members.
Monitoring performance, conducting performance appraisals, and working with the Director of Care to resolve performance concerns.
Leads, coaches and mentors team members; supports continued learning opportunities and fosters a positive working environment;
Collaborate with the Director of Care in the implementation and evaluation of the nursing department initiatives and functions.
Works with the Director of Care, and other leaders as needed, to conduct investigations regarding work performance.
Oversees operational functions including scheduling, recruitment/retention, , return to work plans and performance management (progressive discipline, absenteeism management etc).
Customer Service:
Models and fosters an environment of positive customer service in all aspects of care.
Carries oneself in a professional and personable manner at all times, ensuring appropriate conversation and interactions with all stakeholders at all times.
Ensures residents and/or families are kept up to date on health status, changes, and other pertinent information from MLTC and governing bodies within LTC.
Develop strong relationships with residents/families, becoming a trusted member of the circle of care; ensuring professional relationships are fostered with all team members and residents/families.
Follow-up on all concerns/complaints, in a timely and professional manner while ensuring all policies are followed.
Manage and lead all activities regarding the assigned Resident Home Area
Qualifications:
Education
Bachelor’s degree in Nursing or related science discipline
Masters in Nursing or Healthcare an asset
Professional Certification / Licenses
Current Registration as a RN with College of Nurses Ontario or other regulated health profession
Current CPR
Years and type of experience
Clinical experience as part of an interdisciplinary team in long term care is required.
three to five years experience in supervisory role in long term care is preferred.
Three to five years experience in supervisory or managerial role in health care is required.
Enhanced knowledge of LTCHA, Ont Reg 79/10, Residents Bill of Rights & RAI-MDS and LTC Operations
Demonstrated leadership and communication skills.
Possession of strong motivational, team building and time management skills
