RESIDENT SERVICE COORDINATOR
Position: Resident Services Coordinator – Permanent, Full-Time
Location: Southbridge Cornwall, Cornwall, ON

What You’ll Do:
Conduct resident pre-admission, admission, and ongoing assessments to identify goals, strategies, and expected outcomes.
Participate in interdisciplinary team conferences.
Serve as a liaison with families, providing guidance and support.
Offer counselling or group sessions for residents and families (e.g., orientation, adjustment programs, crisis support, bereavement counselling).
Act as a resource for staff seeking guidance, referrals, or support.
Plan and provide in-service programs for staff, residents, and families.
Maintain accurate records, reports, and statistics in accordance with policies, procedures, and legislation.
Participate in committees related to resident care and facility operations.
Foster open communication with residents, families, and team members to ensure resident needs are met.
Liaise and consult with government officials and professionals regarding resident care.
What We’re Looking For:
Social Services Worker (SSW) Diploma from an accredited college; bachelor’s in social work (BSW) is an asset.
Strong knowledge of community resources, aging, gerontology, and dementia care.
Previous experience in long-term care preferred.
Familiarity with legislation, counselling techniques, and group sessions.
Knowledge of occupational health and safety practices.
Strong English reading, writing, and communication skills.
