Join our team today!Job DescriptionThe Hemodialysis Social Worker is integral to the Social Work professional team and is part of a multi-disciplinary team reporting to the Patient Care Manager, Dialysis, with professional accountability to the Professional Leader, Social Work.Accountabilities:Responsible for the care of patients and families with ESRDParticipate in yearly strategic planning initiatives and corporate professional developmentQualificationsCurrent registration, and in good standing, with Ontario College of Social Workers and Social Service WorkersAbility to provide coverage in critical care and throughout the organization as requiredMasters degree in social work requiredThree to five years of experience working with renal patientsKnowledgeable in community resources (i.e. Kidney Foundation)Membership in CANSW preferredKnowledge and experience working with relevant legislation (i.e. SDM Act, POA, consent and capacity, etc.)Knowledge of the psychosocial impact of chronic illness on the person and the familyKnowledge in Nephrocare preferredFamiliar with Ontario Renal Network guidelines preferredExcellent communication and interpersonal skillsAbility to work as part of a multi-disciplinary teamExcellent organization skills and ability to set priorities as requiredGood critical thinking and problem-solving skills requiredExcellent attendance, punctuality and work recordComputer proficiency preferredMust be able to travel to all Osler sitesMust demonstrate Osler's Values of Respect, Excellence, Service, Compassion, Innovation and CollaborationAdditional InformationHours: Currently days/evenings, 8 hour shifts (subject to change in accordance with operational requirements)Salary:
