What The Role Involves
Professional assessment of patients in the community and in their homes
Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome
Working with the patient on appropriate time-specific, measurable goals and outcomes in collaboration with the care team
Working independently and as an integral member of an interdisciplinary care team
Provide assessment, intervention and discharge planning, within the scope of practice
Demonstrates a clear understanding of Community Resources, including funding
Providing active guidance for workplace Health and Safety
Performing other duties as required
What You Bring
Possess Master’s Degree (or equivalent) degree in Social Work from a recognized university
Current, unrestricted license to practice with Ontario College of Social Workers and Social Service Workers (OCSWSSW)
Proven computer skills, including email, word processing, work scheduling systems
Strong assessment skills and the ability to participate in effective treatment programs
Provide one-on-one therapeutic support with clients
Promotes independence of the client in the home and in the community
A reliable vehicle for some regular travel
Excellent English verbal and written communication skills

Through our Community Nursing Services outreach program, we’ve been organizing staff-led medical care and clinics in countries with poor access to health care since 2009.
