Consider this opportunity. The Change Management Specialist will report to the Manager, Digital Health. This position will play a key role in the successful implementation and adoption of digital technologies such as eReferral and eConsult. This position is accountable for the implementation activities to support Health Service Providers in a variety of hospital and community settings, by applying a structured change management approach and methodology. It will also be important to ensure alignment of work with local digital health priorities to not introduce competing models of change. This regional role focuses on addressing the needs specific to the North East and North West regions. Here is what you will be doing: Primary point of contact for Health Service Providers including solo practitioners, family health teams, family health organizations, and others Work with internal and external partners to apply a structured change management approach and methodology to support digital technology adoption Work with internal and external partners to produce reporting and data analytics to support the overall adoption of digital tools Provide support and formal training to Health Service Providers and partners Work closely with Health Service Providers to strengthen their capacity on digital technologies, data analysis and data use Be an active and visible coach to Health Service Providers and partners Work with Health Service Providers and internal and external partners to create workflows and manage measurement systems to track adoption, utilization and proficiency of users Identify potential risks, points of resistance, performance gaps and develop appropriate mitigation activities Facilitate communication and collaboration with internal and external partners to align local strategies with the provincial vision Work directly with internal and external partners to lead project planning, adoption, implementation, change management and evaluation Document lessons learned and present findings in a clear and concise manner Create and enable reinforcement mechanisms and celebrations of successes Engage Health Service Providers to highlight the value of digital technologies.
