Job Description
The geriatric emergency management (GEM) nurse will focus on frail seniors who are at risk of suffering adverse events, loss of independence and/ or admission to hospital or long term care. GEM nurses provide expert, targeted assessments and treatment in the Emergency Department. As well, they will enhance linkages within the hospital, across Home and Community Care Support Services Central West, and will work in partnership with community services and agencies. In their role as geriatric educators and capacity builders within the Emergency Department, the hospital and the community, GEM nurses help build system capacity to ensure that frail seniors at risk receive the right care in the right place. The successful candidate will work collaboratively with other GEM nurses, Home and Community Care Support Services Central West and long term care nurse practitioners in the Ontario Health Central region to build best practice and facilitate success beyond the organization.
Accountabilities:
Implement standardized protocols to screen and assess high risk seniors
Provide assessments, consultations and advice to help facilitate Emergency Department care including acute symptoms, underlying health conditions and emotional and cognitive status
Initiate appropriate referrals and recommendations including specialist geriatric services
Develop innovative solutions to address concerns to address at-risk patients
Educate patients, families and staff both formally and informally regarding seniors care
Participate in the GEM liaison group and the regional geriatric program
Qualifications
Registered nurse in good standing with the College of Nurses of Ontario
Bachelor's Degree in Nursing or equivalent required
Master's Degree or RN (EC) required
Minimum three to five years of recent emergency or geriatric experience required
Gerontology Nursing certification preferred
Demonstrated excellence in oral and written communication
Demonstrated excellent problem-solving, analytical and teaching skills
Proven ability to practice in a self-directed manner
Demonstrated ability to provide effective professional practice leadership
Advanced knowledge and expertise in nursing assessment, diagnosis and treatment
Knowledge of geriatric medical and psychiatric issues such as dementia, behavioral issues and depression
substance abuse, elder abuse, home safety, falls, mobility, medication issues and wound care
Sensitivity to multi-cultural issues, particularly with the elderly and ability to work with clients of diverse backgrounds
Ability to work independently and be self motivated, as well as work effectively as an inter-professional team member
Excellent organization, interpersonal communication

Demonstrated pattern of good attendance and work record
Required to work in and travel between all Osler sites

Hours: Currently Days, 8/12 hour shifts with a flexible schedule depending on needs of the organization (subject to change in accordance with operational requirements)
Salary:
Minimum: $59.16
Maximum: $66.54
